二尖瓣血流传播速度评价成人房间隔缺损患者介入治疗前后左室舒张功能变化

摘　　要：目的运用彩色M型多普勒超声心动图技术随访观察成人房间隔缺损（ASD）经导管介入治疗前后左室舒张功能变化。方法选择经超声心动图确诊的房间隔缺损患者75例，年龄18～45岁，平均36．4岁。所有患者成功接受ASD介入封堵治疗，并于治疗前及治疗后3月分别进行经胸超声心动图检查，测量指标包括左心室舒张末期内径及容积、射血分数（EF）、二尖瓣血流传播速度（Vp），二尖瓣环组织多普勒（Em／Am）。结果ASD患者治疗后3月左室舒张末期内径及容积较治疗前明显增大（P〈0．001），EF明显改善（P〈0．05），另外，与治疗前相比，Vp和Em／Am比值均明显增高（P〈0．001）。结论二尖瓣血流传播速度指标不受心脏血流动力学的影响，可用于评价ASD患者的左室舒张功能。Objective To evaluate the left ventricular diastolic function in adult atrail septal defect （ASD） patients around transcatheter closure therapy by the propagation velocity （Vp） of mitral valve early flow into the left ventricular cavity measured by color M-mode Doppler echocardiography. Methods Seventy-five patients with ASD diagnosed by echocardiography were studied, aged 18-45 years （mean 36.4 years）. All patients underwent successful interventional therapy and TEE were performed prior to the procedure and in 3 months post therapy. The measurements included [eft ventricular end-diastolic dimension（LVEDD） and left ventricular end-diastolic volume （LVEDV）,ejection fraction （EF）,mitral valve flow propagation velocity （Vp） and mitral annulus velocities of Doppler tissue imaging （Era/Am）. Results LVEDD and LVEDV were significantly enlarged （P〈0. 001） in 3 months post therapy, EF was significantly enhanced （P〈0.05）, and both Vp and Era/Am were significantly increased（P〈0. 001）. Conclusions Vp can evaluate the left ventricular diastolic function in ASD patients.
