摘 要:目的观察逆行胃电刺激(RGES)对健康人胃电活动的影响。方法选择健康志愿者12例。沿胃大弯距幽门5cm处放置一对黏膜电极,进行RGES。观察RGES的长脉冲和串脉冲刺激对多通道胃电图参数的影响,长脉冲对摄食试验中胃电活动的影响,探讨RGES抑制胃动力的电生理学基础。结果所有长脉冲和串脉冲RGES可完全驱动健康人的胃电信号。多通道胃电图分析显示,RGES较大能量的刺激引起ch1和ch4的正常慢波百分比(N%)显著降低,ch1、ch3和ch4的胃电主频(DF)下降,ch1和ch2的主功率(DP)显著降低,胃电慢波耦联率下降。RGES也引起了摄食后ch3的DF显著下降。并且长脉冲对胃电生理的抑制较串脉冲更为明显。结论RGES可抑制胃电活动,尤其以长脉冲作用更明显。这种抑制作用可能是RGES延缓胃排空和减少摄食量的电生理基础。Objective To observe the effects of RGES on gastric electrical activity in healthy humans. Methods Twelve healthy volunteers were recruited in this study. A pair of mucosal gastric electrodes was placed along the great curvature 5cm above the pyloms to generate RGES. To observe the effect of long pulse and serial RGES on multi-channel EGG parameters, the effect of long pulse on gastric electrical activity in food intake test and explore the electrophysiological basis of RGES on the inhibition on gastric motility. Results From multi-channel EGG recording, reduced N% in channel 1 and 4, dominant frequency(DF) declining in channel 1,3 and 4, reduced dominant power(DP) in channel 1 and 2, decreased slow wave coupling resulted from higher energy of RGES were observed. A significant descending of I)F was induced by RGES in channel 3 after water load test. The impairment of gastric electrical activity was more serious induced by RGES of long pulse than that of pulse train. Conclusion Gastric electrical activity was completely entrained by all the settings of serial RGES. GES induces impairment of gastric electrical activity, more severe induced by RGES of long pulse. These impairments may be the underlying mechanism for the delayed gastric emptying and decreased food intake.
