老年高血压患者睡眠状况及其影响因素分析

摘　　要：目的了解老年高血压患者睡眠状况及其影响因素。方法采用匹兹堡睡眠质量指数量表（PSQI）及自行设计的影响因素调查表对100例老年高血压患者进行调查分析。结果76％老年高血压患者存在睡眠障碍，在PSQI总分、睡眠质量、入睡时间、睡眠时间、睡眠效率、睡眠障碍、催眠药物及日间功能障碍各项评分均显著高于国内常模（P〈0．01）。环境（陌生环境、吵闹），生理病理（疾病痛苦、夜间上厕所次数多），心理（对疾病的恐惧、担心医疗费用）和药物（利尿剂的使用、使用某些降压药引起的不良反应）是影响其睡眠质量的重要因素。结论老年高血压患者睡眠质量较差，并与多种因素有关，应及早采取有效的护理措施控制影响睡眠的各种因素，提高患者睡眠的质量。Objective To investigate the sleep condition and its influence factors in old patients with hypertension. Methods A total of 100 old patients with hypertension were investigated by Pittsburgh sleep quality index （PSQI） and self-designed questionnaire of influence factors. Results 76 percents of the old patients with hypertension had sleep disorders.The total score of PSQI,sleep quality,time take to fall asleep,hours of sleep,sleep efficiency,sleep disorders,hypnotic drugs and every score of daytime dysfunction were significantly higher than domestic norm（P 〈 0.01 ）.Surroundings（strange circumstances or noisiness）,physio-pathology （disease distress or high frequency of going to toilet during nighttime）,psychological situations（ the fear to the disease or worrying about medical fees） and drugs（use of diuretics or the adverse effects of some antihypertensive agents） were the important factors affecting sleep quality. Conclusions Old patients with hypertension have poor sleep quality,which relates with multiple factors.Valid nursing interventions should be adopted in order to control the various factors which affect sleep quality to improve patients＇ sleep quality.
