IL-1与选择素CD62L,CD62P,CD62E在小儿早期颅内感染性疾病中的临床意义

摘　　要：目的研究白细胞介素及选择素在小儿急性细菌性脑膜炎及病毒性脑膜炎的临床诊断、治疗、病情转归和判断预后的应用价值。方法以ELISA法分别检测小儿急性细菌性脑膜炎及病毒性脑膜炎患者治疗前以及正常对照组脑脊液白细胞介素1(Interleukine-1,IL-1);用流式细胞仪测定选择素CD62L,CD62P,CD62E含量。并探讨各指标与颅内感染鉴别诊断与预后的关系。结果小儿急性细菌性脑膜炎及病毒性脑膜炎患者治疗前脑脊液IL-1,选择素CD62P,CD62E,CD62L含量较对照组明显升高(P<0.01);急性细菌性脑膜炎及病毒性脑膜炎患者治疗后以上各指标明显下降(P<0.05)。结论脑脊液IL-1,选择素CD62L,CD62P,C62E含量水平的变化与颅内感染的临床变化有较密切的关系,可以作为大脑炎症病情变化的监测指标和预后判断指标,同时也为在脑膜炎发病早期使用抗黏附分子抗体及药物治疗提供临床实验依据。Objective To evaluate the significance of the expression of Interleukine-1 （IL-1） and Selectin in children with acute bacterial meningitis and virus meningitis. Methods The expression of IL-1 in cerebrospinal fluid （CSF） in children suffered from acute bacterial meningitis and virus meningitis before treatment and contrals CD62L,CD62P,CD62E were detected by flow cytometry, the explore the correlation between these indexes and the differential diagnosis,prognosis of intracranial infection. Results The CSF levers of IL-1,CD62P,CD62E,CD62L were higher apparently in cases with acute bacterial meningitis and virus meningitis than those in contral （P〈0.01）. Moreover,after treatment,all indexes obviously decreased compared with before treatment （P〈 0. 05）. Conclusion The changes of IL-1 and Selectin CD62L, CD62P, CD62E related to the seriousness of intracranial infection,which may reflect the seriousness and prognosis of cerebral inflammation. In addition ,this study may provide some experimental evidences for application of adhesion molecule antibody in the early onset of intracranial infection and its drug therapy.
