摘 要:目的分析中国西北地区十家教学医院2006~2007年度临床分离嗜血杆菌的耐药状况。方法常规方法培养分离医院内感染病原菌,并应用半自动或全自动细菌鉴定分析仪鉴定到种,药敏试验方法按CLSI规定的标准进行。采用WHONET5.4软件进行数据统计分析。结果2006~2007年度西北地区10家教学医院共分离出嗜血杆菌356株,其中流感嗜血杆菌112株,副流感嗜血杆菌134株,溶血性嗜血杆菌89株,副溶血性嗜血杆菌21株。药敏监测结果显示,流感嗜血杆菌、副流感嗜血杆菌均对复方新诺明、氨苄西林、环丙沙星高度耐药,耐药率分别为72.4,42.6,34.6和51.1,32.1,35,对亚胺培南、美罗培南完全敏感;溶血性嗜血杆菌、副溶血性嗜血杆菌均对氨曲南、氨苄西林、头孢曲松高度耐药,对左氧氟沙星、头孢呋新、氨苄西林/舒巴坦较敏感。结论嗜血杆菌对多种抗生素的耐药呈现明显上升趋势,开展不间断的完整的耐药监测,合理使用抗菌药物,对于控制耐药菌株快速上升具有重要意义。Objective To analyse of 2006-2007 annual drug resistance anainst Haemophilus isolated from ten teaching hospitals in Northwest of China. Methods Isolates were cultured by routine methods and identified by the semi-automatic or automatic bacteria analysis system. Antibacteral susceptibility tests were carried out according to CLSI standards. Monitering data were delivered to Ministry of Health National Antimicrobial Resistant Investigation Net (Mohnarin) and data analysis was performed by using WHONETS. 4 software. Results 356 strains Haemophilus were isolated from ten teaching hospitals in Northwest of China, in which Hemophilies influenzae account for 112 strains (31. 46%), Hemophilus parainfluenzae 134 strains(37.64%), H. haemolyticus 89 strains(25%) and H. parahaemolyticus 21 strains(5.90%). The monitoring results showed that both H. influenzae and H. parainfluenzae were highlyresistant against sulfamethoxazole, ampicillin and ciprofloxacin with resistance rate 72.4%,42. 6%, 34.6 % and 51.1 %, 32.1%, 35 % respectively, while both were totally susceptibile to Imipenem and Meropenem ; both H. haemolyticus and H. parahaemolyticus were highly resistant against aztreonam,ampieillin and eeftriaxone and susceptibile to levofloxacin,eefuroxime,ampicillin/sulbaetam. Conclusion Antibacteral resistance for Haemophilus is ascending. It is significant to control the rapid emerging of resistant strains by improving the isolation and susceptibility test of Haemophilus in respiratory tract and carrying out complete supervision of the antibacteral resistance.
