IL-6水平检测与重症并发感染者的相关性研究

摘　　要：目的探讨血清IL-6水平在重症并发感染及非感染者体内的变化规律及意义。方法放射免疫分析法检测40例APACHE评分≥12分重症并发感染的病人,30例正常对照组,30例重症非并发感染病人血清IL-6水平并进行比较和相关性分析。结果重症并发感染组血清IL-6水平比重症非并发感染组升高,但该两组的差别无统计学意义(P>0.05),他们与正常对照组比较均升高,差别有统计学意义(P<0.01)。重症并发感染组血清IL-6水平与病人的预后呈正相关,与APACHE评分呈正相关,与白细胞数均值呈正相关。40例重症并发感染患者中有20例死亡,他们的血清IL-6水平较生存病例高。30例重症非并发感染病人的血清IL-6水平与APACHE评分呈正相关。IL-6水平的灵敏度和特异度分别为80和90,APACHE评分为65和80。结论重症患者无论并发感染与否其血清IL-6水平均升高,其升高反应了损伤的严重程度,与预后有较好的相关性,IL-6的灵敏度和特异度更高。Objective To explore reveal regularity and significance of serum interleukin-6（IL-6） levels in serious patients with and without infections. Methods IL-6 levels were detected in 40 APACHE Ⅱ score over 12 serious patients with infections, 30 normal controls and 30 serious patients without infections with radio-immunity analysis assay. Then compared the IL-6 levels and the correlation among them. Results Both of the serious patients groups were significantly higher than the normal control group（P〈0.01）,but there were no significant differences between the serious patients groups（P〉0. 05）. In the group of serious patients with infections,there was a positive correlation between the IL-6 levels and the prognosis,that between the IL-6 levels and APACHE Ⅱ score,and that between the IL-6 levels and the average of white blood cell count too. Among 40 serious patients with infections, 20 patients were dead,and their IL-6 levels were higher than those of the survivors. In 30 serious patients without infections,there was a positive correlation between the IL-6 levels and APACHE Ⅱ score. Sensitivity and specificity of the IL-6 levels were 80% and 90G ,and APACHE II score were 65% and 80%. Conclusion The IL-6 levels are elevated between the serious patients with and without infections. It appear to reflect the severity of disease,and there is a good correlation between the IL-6 levels and prognosis. The IL-6 levels have higher sensitivity and specificity.
