老年重症乳糜尿的手术治疗

摘　　要：目的评价肾蒂淋巴管结扎术治疗老年重症乳糜尿的临床疗效。方法对16例确诊为重症乳糜尿的老年患者行肾蒂淋巴管结扎术。结果术后3 d乳糜定性阴性14例(87.5%)。术中肾静脉损伤1例,术后肉眼血尿3例,严重肺部感染1例,切口感染2例。12例随访0.5～5年,1例复发(8.33%);11例B超复查,2例手术侧肾脏轻度萎缩;营养状况和体力明显改善。结论肾蒂淋巴管结扎术治疗老年重症乳糜尿疗效确切。注重围手术期管理、提高手术技巧、加强并发症防治,可有效提高手术成功率。Objective To evaluate curative effect of severe chylous urine treated with renal pedicle lymphatic disconnection in old patients. Methods 16 patients with final diagnosed severe chylous urine underwent renal pedicle lymphatic disconnection. Results Regular lab test for chylous urine was taken 3 days after operation. 14 cases returned negative. 1 case was injury of renal vein during operation; 3 cases were gross hematuria after operation; 1 case was serious pulmonary infection; 2 cases were infection of incisional wound. 12 cases were performed for 6 months to 5 years follow-up. 1 case was found relapse 5 months after operation. 2 of 11 cases rechecked by B typed ultrasound were slight renal atrophy. Conclusions Renal pedicle lymphatic disconnection is a effective mean for treatment of severe chylous urine in old patients.
