类风湿性关节炎患者血清可溶性细胞间粘附分子-1测定与核素骨显像相关性研究

摘　　要：目的研究类风湿性关节炎（RA）患者血清可溶性细胞间粘附分子-1（sICAM-1）水平变化及其与^99Tc^m-MDP骨关节显像的相关性，探讨在本病早期诊断中的意义。方法用ELISA法检测达标组（23例）、非达标组（28例）及对照组（20例）血清sICAM-1水平。以^99Tc^m-InMDP为示踪剂．对上述3组受检者行核素骨关节显像，进行肉眼定性及半定量分析。结果达标组、非达标组血清sICAM-1水平明显高于对照组（P〈0．01和P〈0．05），达标组与非达标组比较，血清sICAM-1水平差异无统计学意义（P〉0．05）。非达标组、达标组血清sICAM-1水平与手足关节指数有相关性（P〈0．01或P〈0．05）。达标组、非达标组手、足、腕、踝病变关节放射性浓聚明显高于对照组。结论检测血清sICAM-1水平及^99Tc^m-MDP骨关节显像定性、半定量分析，有助于类风湿性关节炎患者，尤其是早期进行诊断及判断病情活动程度，改善预后。Objective To study serum levels of soluble intercellular adhesion molecule -1 （ sICAM-1 ） and its correlation with accumulation degree of 99Tc^m-MDP on bone and arthritis and to study the role of sICAM-1 in the diagnosis of patients with early RA. Methods Serum levels of slCAM-1 were measured by immunoenzymetic assay （ ELISA ）. 99mTc-MDP scintigraphy was carried out for the same patients. The significant test between serum levels of slCAM-1 of three groups was performed with analysis of variance ; Correlations between serum levels of sICAM-1 and index of semiquatitive ^Tc^m-MDP were obtained using sperman＇s ranks correlation method. Results Compared with control group, serum levels of sICAM-1 were significantly higher in standard group （ group A ） （ P 〈 0.01 1 and non-standard group （ group B ） （ P 〈 0.05 ）. No significant difference was found between group A and B （ P 〉 0.05 ）. Serum levels of slCAM-1 were positively correlated with ^99Tc^m-MDP scintigraphy. Most patients of non-standard group and standard group showed visible medium-high uptake of ^99Tc^m-MDP. While control group had not obviously uptake of ^99Tc^m-MDP. Conclusion Serum levels of sICAM-I, semiquatitive ^99Tc^m-MDP scintigraphy, accumulation degree of radiopharmuceuutial on bone and arthritis are helpful for diagnosis of early rheumatoid arthritis and judgement of the clinical status and improvement of the prognosis.
