摘  要:【目的】探讨胰岛素抵抗(IR)、血脂代谢异常与冠心病(CHD)的关系。【方法】调查40例CHD患者及40例正常对照组的一般生活史,使用ELISA法检测空腹真胰岛素(FTI)、空腹胰岛素原(FPI)水平,使用放射免疫法(RIA)检测空腹血清免疫活性胰岛素(FIRI),检测观察对象空腹血糖(FBG)、餐后2 h血糖(2h-PG)、血脂、脂蛋白水平等,并计算胰岛素敏感指数(insulin sensitivity index,ISI)评价胰岛素抵抗程度。【结果】CHD组与对照组比较,2h-PG、FIRI、FTI、FPI、TG、TC、LDL-C、apoB显著升高,ISI、HDL-C显著降低(P<0.05)。CHD组中FTI、FPI与年龄、BMI、血糖和血脂均有独立的相关关系。【结论】CHD存在IR和高胰岛素血症(HIS),FTI、FPI分别与血糖、血脂等CHD危险因子呈群聚关系,IR及HIS可能通过引起脂质代谢紊乱而与CHD密切相关。[Objective]To explore the relationship among insulin resistance(IR),dyslipidemia and coronary heart disease(CHD).[Methods]The life history of 40 patients with CHD and 40 healthy people was investigated.Fasting true insulin(FTI) and fasting proinsulin(FPI) were measured by ELISA assays and fasting immunoreactive insulin(FIRI) was detected by RIA.Fasting blood glucose(FBG),2h postprandial plasma glucose(2h-PG),serum lipids and lipoprotein were determined in each individual.Insulin sensitivity index(ISI) was used to evaluate the level of IR.[Results]The levels of 2h-PG,FIRI,FTI,FPI,TG,TC,LDL-C and apoB were significantly higher in patients with CHD than those in controls,while the levels of ISI and HDL-C were lower in patients with CHD than those in controls(P〈0.05).There were significant independent correlations between FTI,FPI and age,BMI,plasma glucose and serum lipids.[Conclusion]People with CHD have IR and hyperinsulinemia(HIS) and there is an independent association between proinsulin and dyslipidemia.IR and HIS can cause the disorders of lipid metabolism,and promote the progress of CHD.
