摘 要:[目的]观察妊娠肝内胆汁淤积症(ICP)时,孕妇血及脐血中一氧化氮(NO)水平与胎儿脐动脉血流的关系,以进一步探讨ICP时胎盘循环阻力增高的原因并对因护理。[方法]对35例ICP孕妇(ICP组)及32例正常孕妇(对照组)用镉还原显色法测定母血及脐血NO水平;超声多普勒测定胎儿脐血流S/D值。[结果]ICP组母血、脐血NO水平均明显低于对照组(P<0.01);两组母血、脐血NO水平均有相关关系(r分别为0.5 4 8 1、0.820 6,P<0.01),ICP组脐动脉血流S/D值明显高于对照组(P<0.01),母血、脐血NO水平与S/D值呈负相关(r分别为-0.3415、-0.426 1,P<0.05或P<0.01)。[结论]ICP时母血、脐血NO水平降低对胎儿-胎盘循环阻力增加有重要的作用。Objective: To observe the relation between blood nitrogen monox- idum (NO) and cord blood level of gravida and blood flow of umbilical artery of fetus, so as to probe into causes of enhancing of placental circulation resistance further and corresponding nursing care methods of gravida with intrahepatic cholestasis of pregnancy (ICP). Methods: A total of 35 patients with ICP (ICP group) and 32 health pregnant women (control group) were selected. The blood NO level of mother and cord blood was determined with cadmium reduction method. And the S/D value of cord blood flow of fetus was determined with ultrasound Doppler. Results: The level of NO in mother blood and cord blood in ICP group was lower than that of control group (P〈0. 01). There was positive correlation between NO level in mother blood and in cord blood in both groups (r=0. 548 1 and 0. 820 6 respectively, P〈0.01). And the S/D value of umbilical artery blood flow in ICP group was much higher than that of control group (P〈0.01). There were negative correlations between NO level in mother blood and cord blood and the S/D value (r=-0.341 5, P〈0. 05 ;r=-0. 426 1, P〈0. 01). Conclusion:Among pa- tients with ICP, the decrease of NO in mother blood and cord blood played an important role in the high resistance of fetal--placental circulation.
