超声诊断子宫角妊娠17例临床分析

摘　　要：目的探讨超声在子宫角妊娠诊断、鉴别诊断中的临床应用价值。方法对17例手术病理检查证实为子宫角妊娠患者的声像图表现进行回顾性分析并行超声分型。结果17例子宫角妊娠术前超声检出妊娠囊型8例(8/10,1例误诊为宫外孕,1例因子宫及附件超声检查无阳性发现漏诊);检出包块型2例(2/3,误诊为宫外孕1例);检出破裂型1例(1/4,误诊为宫外孕3例)。超声与病理诊断结果对比显示,术前超声正确诊断子宫角妊娠11例(妊娠囊型8例、包块型2例、破裂型1例),误漏诊6例(妊娠囊型2例、包块型1例、破裂型3例)。结论超声诊断子宫角妊娠简便安全,结果可靠,但包块型和破裂型误诊发生率仍较高。Objective To explore the ultrasound diagnosis, differential diagnosis and the clinical values of uterine corner pregnancy by ultrasound. Methods The ultrasonograms of uterine corner pregnancy patients diagnosed by pathology and surgery were retrospectively analysed and classified. Results In seventeen cases diagnosed with uterine corner pregnancy by pre-surgery ultrasound ： （ 1 ） Eight of ten cases were diagnosed with pregnant sac model, one case was misdiagnosed ectopic pregnancy and one was not positively founded by ultrasound. （2） Two of three cases diagnosed as mass model, and one case of misdiagnosed ectopic pregnancy. （3） One of four cases diagnosed ruptured model and three cases of misdiagnosed eetopic pregnancy. Compared to pathological diagnosis, there are eleven cases （ including eight cases of pregnant sac model, two cases of eetopic pregnancy and one case of ruptured model） correctly diagnosed by pre-surgery ultrasound and six cases （ including two cases of pregnant sac model, one case of ectopic pregnancy and three cases of ruptured model） were misdiagnosed. Conclusions The ultrasound is typical, safe and reliable diagnostic method for uterine corner pregnancy, but diagnosis of mass model and ruptured mode is still open to questions.
