摘　　要：目的探讨原发性肾病综合征(PNS)患儿血CD30阳性细胞和IL-6、IL-10的变化及其临床意义。方法将56例初发的单纯性PNS患儿根据泼尼松足量、中程疗法治疗8周内尿蛋白是否转阴分为激素敏感组和非激素敏感组,同时另选20例健康儿童作为对照组,分别检测外周血单个核细胞(PBMC)中CD30阳性细胞百分率及PBMC培养上清液中IL-6、IL-10的浓度。结果激素敏感组活动期患儿CD30阳性细胞百分率、IL-6和IL-10浓度均显著高于对照组(均P<0.01);激素敏感组缓解期和激素非敏感组患儿各指标与对照组比较差异均无统计学意义(均P>0.05)。结论CD30阳性细胞和IL-6、IL-10可能在激素敏感性PNS发病中具有一定的作用;在PNS治疗前和治疗中,适时检测CD30阳性细胞和IL-6、IL-10水平可能对预测激素疗效及调整治疗方案具有重要意义。Objective To investigate the changes and clinical significance of blood CD30^＋ cell and interleukin-6 （IL-6）, interleukin-10（IL-10）levels in children with primary nephrotic syndrome （PNS）. Methods Fifty six PNS patients were divided into two groups （steroid sensitive group and steroid resistive group） according to positive or negative proteinuria after orally taking prednisone in 8 weeks. Twenty healthy children were served as the control group. The percentage of CD30^＋ cells and levels of IL-6,1L-10 in peripheral blood mononuclear cell （PBMC） were assayed at admission and alleviation by flow cytometer and ELISA respectively. Results There were a higher percentage of CD30^＋ cells and higher levels of IL-6 and IL-10 in steroid sensitive group as compared to healthy control group （P〈0.01） at admission. No significant differences were found in the percentage of CD30^＋ cells and levels of IL-6 and IL-10 in steroid sensitive group as compared to healthy control group at alleviation, and in steroid resistive group as compared to healthy control group. Conclusion The determination of CD30^＋ cell ratio and IL-6, IL-10 Levels in PBMC before and during treatment can be used to predict the curative effect of steroid therapy.
