摘 要:目的探讨IL-1受体拮抗剂(IL-1ra)缓释系统抑制后发性白内障发生的可行性和有效性。方法将30只健康日本大耳白兔(30眼)晶状体囊外摘出术后随机分为A组(空白对照组)、B组(囊袋内植入空白缓释系统)及C组(囊袋内植入IL-1ra缓释系统)。术后8周对3组兔眼进行裂隙灯显微镜、角膜内皮镜检查及组织病理学、电镜观察,并检测房水中IL-1的质量浓度。结果术后12周A、B、C组发生晶状体后囊膜混浊分别为10、10、3眼,差异有统计学意义(P<0.01);术前及术后C组角膜内皮细胞计数与对照组比较差异无统计学意义(P>0.05);术后1、3、7、14、28、56d,C组IL-1质量浓度与A组和B组间差异有统计学意义(P<0.01);术后3、7、14、28、56d,A组和B组比较IL-1质量浓度差异无统计学意义(P>0.05)。光镜和电镜下C组兔眼晶状体后囊膜细胞增生不活跃,未发现眼内毒性反应。结论后房内植入IL-1ra缓释系统,可明显抑制房水中的IL-1质量浓度,可有效抑制后发性白内障的发生。Objective After cataract is a main cause influencing vision following extracapsular cataract extraction. In this paper,IL-1 receptor antogonist (IL-1ra) drug delivery system (IL-1ra DDS), an interleukin 1 antagonist, was developed and applied in the experimental animals for the prevention of after cataract. Methods Thirty Japanese rabbits(30 eyes) received extracapsular cataract extraction were randomly divided into 3 groups:The rabbits in group A received only extracapsular cataract extraction as control group,and carrier drug delivery system without or with IL-1ra DDS was implanted into capsular bag after extraeapsular cataract extraction in the rabbits of group B and group C respectively. All eyes were examined by slit-lamp microscope,and the aqueous humour was collected at 1 day, 3,7,14,28 and 56 days after operation. IL-1ra concentration in aqueous humour was detected by ELISA. Corneal endothelial cells were counted. Results In the 12th week after the operation, the eye numbers developed posterior capsule opacification (PCO) was 10,10 and 3 in group A, B and C respectively with a significant difference between control group and IL-1ra DDS group(P 〈 0. 05). There was no significant difference in the number of corneal endothelial ceils between group C and A or B in the 28th day after operation and before operation( P 〉 0.05 ). No toxic reaction on cornea/ endothelial cells was indicated after usage of IL-1ra DDS. The content of IL-1 in group C was significantly lower than group A and B, showing a statistically significant difference (P 〈 0.05 ). No or only sporadic lens epithelial cells and fibrin sticking were found at posterior capsule of lens in group C under the light and electron microscope, and severe PCO was seen in group A and B. No intraocular hemorrhage occurred during the follow-up period in various groups. Conclusion The implantation of IL-1 ra DDS into in vivo capsular bag can elevate the IL-1 ra concentration in aqueous humour and remain for a long period. This
