摘 要:目的采用组织多普勒成像技术,计算心肌作功指数,评价尿毒症患者的右心功能的变化。方法尿毒症组35例,根据患者的左室质量指数(LVMI)及有无心包积液(PE),将其分为LVMI增大组19例及LVMI增大伴PE组16例。正常对照组25例。记录三尖瓣环的隔瓣处、前瓣处、后瓣处的右室等容收缩时间(ICT)、右室等容舒张时间(IRT)及右室射血时间(ET),计算右室心肌作功指数(MPI)。结果尿毒症LVMI增大伴PE组及尿毒症LVMI增大组LVMI均大于正常值,IRT、ICT均较正常对照组延长,ET均较正常对照组缩短(P<0.05)。右室心肌作功指数明显增大,较正常对照组增大(P<0.01);而且尿毒症LVMI增大伴PE组指数增大幅度比尿毒症LVMI增大组大(P<0.05)。右室MPI与IRT,ICT,(IRT+ICT)间有显著性正相关关系(r分别为0.765、0.563、0.874,P<0.001),与ET间有显著性负相关关系(r=-0.711,P<0.001)。结论组织多普勒心肌作功指数,为评价尿毒症患者的右心室收缩、舒张功能提供了新的定量指标。Objective To assess the right ventricular function in patients with uremia using myocardial performance index (MPI) on pulsedwave tissue Doppler echocardiography (TDE). Methods 35 patients with uremia and 25 healthy subjects were included in this prospective study. According to the left ventricular mass index (LVMI) and pericardial effusion (PE) measured on conventional echocardiography, the patients with uremia were classified into two groups : the increased LVMI group and the increased LVMI with PE group. In order to calculate right ventricle MPI on TDE, isovolumic contraction time (ICT), isovolumic relaxation time (IRT), and ejection time (ET) were measured at three different sites in the tricuspid annulus: septum, anterior and posterior. The mean MPI value was got from the sum of these MPI divided by 3. Results The mean ICT and IRT were prolonged and ET was shortened in the two disease groups. MPI was higher in two disease groups than that of the control group, and in the increased LVMI with PE group, it was higher than in the simple increased LVMI group. There was significant difference in the mean MPI between the two disease groups and the control group (P〈0.01) / MPI was positively correlated with IRT, ICT, and (IRT+ICT) (r=0. 765, 0. 563, 0. 874, P〈0. 001, respectively), and negatively correlated with ET (r =- 0.711, P〈0. 001). Conclusions MPI may provide a novel, noninvasive method for clinical research on right ventricle myocardial performance of patients with uremia.
