联合微创手术治疗大隐静脉曲张

摘　　要：目的:考察大隐静脉高位结扎剥脱联合微创切除电凝术的可行性及其疗效。方法:选择我科2003年3月—2006年6月的185例大隐静脉曲张患者施行治疗,常规硬膜外麻醉或腰麻,穿刺内踝上方大隐静脉主干,以16号套管针穿刺静脉,套管针全程进入静脉,导入电极,设定好功率,边烧边退,同时助手沿大隐静脉沿途压迫,使静脉腔闭合,烧灼完毕后,弹力绷带加压包扎。结果:185例患者无短期术后并发症发生,单侧肢体平均手术时间35～70min,多数效果良好,5例复发,再行大隐静脉高位结扎浅静脉剥脱术而痊愈。结论:大隐静脉高位结扎剥脱联合微创切除电凝术对大隐静脉曲张治疗效果良好,但其远期疗效尚待有进一步观察。Objective： Discuss the feasibility and therapeutic effect of exalted ligation combined electrocoagulation operation for varicose vein of lower extremity. Methods： We selected 185 patients in our department by using exalted ligation combined electrocoagulation operation.After epidural anesthesia or lumbar anesthesia we stick through the trunk of great saphenous vein superior the medial malleolus, then turn on the electrocoagulation;enactment power, pull the cauterize at the same time while the assistant press the leg along the great saphenous vien to make the lumen of vein occlusion.About seven days after the operation patient can leave the hospital. Results ： The 185 patients did not have short-term complication after the operation.Average surgery time is 55-70 minutes in single leg.The most effects are good,only 5 patient retured,then they recovered after high ligation and stripping.Conclusion： exalted ligation combined electrocoagulation treatment to patients with varicose vein of lower extremity is effective,but its prospecdtive efficacy need further observation.
