摘  要:目的通过对髌骨骨折术后病例进行分析研究,探讨不同类型的髌骨骨折最佳固定方法。方法髌骨骨折手术内固定364例,其中钢丝环扎78例,AO张力带固定110例,可吸收钉25例,Ni-Ti形状记忆合金聚髌器151例。根据术后6个月膝关节功能、X线表现进行综合评价,对比分析术后疗效。结果364例均获随访,平均随访时间2.2年。发生各种并发症30例,其中钢丝环扎术后9例,张力带13例,可吸收钉3例,Ni-Ti形状记忆合金聚髌器5例。再手术10例,所有骨折均临床愈合。结论髌骨骨折应根据骨折类型选择不同的内固定方式,Ni-Ti形状记忆合金聚髌器具有较低的并发症发生率,适用于各种类型髌骨骨折。Objective To explore the best fixing method of different types of patella fracture by analyzing postoperative complications. Methods The retrospective analysis was done according to the information and records of the 364 patients with patellar fractures who were cured by us in recent 9 years. The steel wire fixing was used in 78 patients, AO- tension band fixing 110, poly- L- lactic acid screw fixing 25, shape-remembering alloy claw fixing 151. The curative effects were judged by the functions of the patients' knee joints and the results of X - ray examinations in different periods after operation. Results The average follow up time was 2.2 years. A- mong the 364 patients, 30 had postoperative complications, including 9 with steel wire fixing, 13 with AO- tension band fixing, 3 with poly- L - lactic acid screw fixing, and 5 with shape - remembering alloy claw fixing. Analysis and comparison for the treatment of 4 types of fixing methods and the relevant curative effects were made. Conclusion The operation techniques should be chosen according to the type of fracture. Ni - Ti shape - remembering alloy claw is the suitable for the treatment of all kinds of patella fracture.
