摘 要:目的探讨头孢他啶对不同浓度大肠埃希菌内毒素释放的影响及常用清热解毒中药注射剂体外抗内毒素活性。方法采用二倍稀释法测定头孢他啶对大肠埃希菌的MIC,并确定清开灵、双黄连和喜炎平注射剂临床常用剂量对MIC的影响;50×MIC头孢他啶对处于对数生长期不同浓度大肠埃希菌内毒素产生的影响;清开灵、双黄连和喜炎平注射剂与头孢他啶联合应用时抗内毒素作用的研究。结果头孢他啶对大肠埃希菌的MIC为0.125μg/ml,清开灵、喜炎平注射剂对头孢他啶的MIC无影响,双黄连注射剂可显著提高头孢他啶MIC;头孢他啶于高菌液浓度时可诱导内毒素释放,在低菌液浓度时抑制内毒素释放;中药注射剂与头孢他啶联合应用时,加药顺序对内毒素的产生有一定影响,当清开灵、喜炎平注射剂在头孢他啶之后应用时,可显著降低内毒素含量,喜炎平的效果优于清开灵注射液。结论头孢他啶对大肠埃希菌内毒素释放的影响与菌液浓度有关。清开灵、喜炎平注射剂具有抗内毒素作用,与头孢他啶联用时,用药顺序对内毒素的消除有一定影响。Objective To study the factors that influence the endotoxin release induced by ceftazidime in different Escherichia coli concentrations, and to explore the anti-endotoxin effects of 3 kinds of Chinese medicine injections, namely Qingkailing injection, Shuanghuanglian injection and Xiyanping injection in vitro. Methods Two-fold dilution to determine the MIC of ceftazidime, and determine if the MIC could be altered by 3 kinds of injections when used in routine dosages ; to study if ceftazidime (at 50 × MIC) could affect the production of endotoxin while added to different concentrations of Escherichia coli which in logarithm phase; to determine the anti-endotoxin activity of the three injections when used concomitantly with ceftazidime. Results The MIC of ceftazidime was 0. 125μg/ml, Qingkailing injection and Xiyanping injection had no effect on it, but Shuanghuanglian injection could lead it to increased; at high Escherichia coli concentrations (2.5 × 10^4-2. 5 ×10^5 CFU/ml), ceftazidime induced a great deal of endotoxin release, but at low concentrations (2.5 × 10-2.5 ×10^3 CFU/ml), ceftazidime inhibited endotoxin release; the using sequence of Chinese medicine injections and ceftazidime could affect endotoxin release. When Qingkailing injection and Xiyanping injection used after giving ceftazidime, they showed anti-endotoxin effect, and Xiyanping injection was the better. Conclusion The endotoxin releasing activities of ceftazidime were not only related to the bacterial concentrations but also influenced by the sequence usage of Qinakailing or Xiyanpang injections.
