硬膜外注射配合手法复位治疗急性腰椎间盘突出症的临床研究

摘　　要：目的:探讨硬膜外注射配合手法复位对急性腰椎间盘突出症的疗效.方法:将1000例腰椎间盘突出患者随机分为硬膜外注射组500例和常规治疗组500例,疗程3～4wk,治疗前及治疗后1d,1wk和疗程结束后应用目测类比评分法(VAS)对疼痛进行评估,对症状、体征进行临床评定.结果:常规组和硬膜外注射组治疗前VAS评分分别为(8.6±1.4)和(8.8±1.4)分,常规治疗后第1日及第1周VAS评分常规组分别为(8.2±0.6)和(5.3±0.9)分,硬膜外注射组分别为(5.5±0.9)和(2.7±1.1)分,两组止痛效果比较,差异有统计学意义(P<0.05).结论:硬膜外注射配合手法复位对腰椎间盘突出症有较好的临床疗效.AIM：To study the therapeutic effect of epidural injection and manual reduction on acute lumbar disc herniation. METHODS： 1000 patients diagnosed with acute lumbar disc herniation were equally divided into the epidural injection group （ n = 500） and the traditional treatment group （ n = 500 ）. The clinical symptoms and signs as well as the scores of visual analogue seale（VAS） were assessed before and on the 2nd day, 1 week and 3 -4 weeks after the treatment. RESULTS： Before treatment, the VAS score was （ 8.6 ±1.4） and （ 8.8 ±1.4） in the traditional treatment group and the peridural injection group respectively, and, at 1 day and 1 week after treatment, it was （ 8.2 ± 0.6 ） and （ 5.3 ± 0.9 ） in the traditional treatment group, and （5.5 ±0.9）and （2.7 ± 1.1） in the epidural injection group respectively. There was significant difference in analgesic effects between the 2 groups （ P 〈 0.05 ）. CONCLUSION ： Epidural injection combined with manual reduction is an effective therapeutic option for the treatment of patients with lumbar disc herniation.
