摘 要:目的评价18F-FDG PET/CT对分化型甲状腺癌转移的诊断价值。方法20例经手术确诊的分化型甲状腺癌患者,并已行1次或多次131I治疗,行18F-FDG PET/CT检查后,与治疗后全身131I扫描结果比较,比较其在初次及多次131I治疗后检查发现病灶的情况,并对两种检查的一致性进行对比。结果20例患者中,PET/CT发现病灶数与131I全身扫描发现病灶数无显著性差异;但结果显示在首次治疗后131I扫描发现病灶数少于PET/CT,第2、3次治疗后131I扫描发现病灶数多于PET/CT,多次治疗后131I扫描发现病灶数又少于PET/CT。结论18F-FDG PET/CT和治疗后131I全身扫描在分化型甲状腺癌转移诊断方面均有较高价值,临床应用中应视具体情况选择,两者配合应用具有较高临床诊断价值。Objective To investigate the diagnostic value of PET/CT in differentiated thyroid carcinoma (DTC). Methods Twenty patients with surgically proved DTC underwent both PET/CT and 131 I whole body scan. The meta static lesions displayed by ^131I whole body imaging were counted and compared with those shown on PET/CT. Results All 20 patients were divided into 3 groups: group A underwent 131^I whole body scan once, group B twice or thrice, and group C more than thrice. The metastatic lesions displayed by PET/CT in group A and C were more than those in group B, but without significant difference. Furthermore, a consistency analysis of the two methods showed good accordance. Conclusion Both PET/CT and 131I whole body scan have important diagnostic values for DTC. They can be complementarily used in clinic.
