摘　　要：目的探讨肾小球足细胞相关蛋白nephrin和podocin在V型狼疮性肾炎(V-LN)发病中的作用。方法检测V-LN组11例和特发性膜性肾病(I MN)组8例患者的尿常规、24-h尿蛋白定量、尿补体C3及纤维蛋白原降解产物(FDP)、血脂、血清补体及肝肾功能,并行肾组织活检间接免疫荧光染色,激光共聚焦显微镜观察nephrin、podocin表达的变化。结果24-h尿蛋白定量、血清白蛋白、肌酐、尿补体C3两组间无统计学差异;V-LN组血脂、血清补体C3、C4及尿FDP均低于I MN组;nephrin、podocin表达两组均减弱,但I MN组更明显。结论Nephrin、podocin表达减弱可能参与V-LN蛋白尿的发生,但V-LN表达减弱不及I MN明显,推测V-LN蛋白尿的发生可能还有其他机制参与。Objective To investigate the expressions of nephrin and podocin in type V lupus nephritis.Methods Eleven patients with type V lupus nephritis（V-LN） and 8 patients with idiopathic membranous nephritis（IMN） were enrolled in the study.Twenty-four-hour urine protein excretion,serum albumin,creatinine,triglyceride,total cholesterol,serum C3,C4,urine C3 and FDP were observed in two groups.Glomerular lesions were measured by light and electronic microscopy.The expressions of nephrin and podocin were detected by indirect immunofluorescent staining.Results Compared to IMN group,24-hour urine protein excretion and the concentrations of serum albumin,creatinine,and urine C3 were not significantly different.Triglyceride,total cholestorel,serum C3,C4 and urine FDP were significantly lower in V-LN group than those in IMN group（P〈0.05）.By indirect immunofluorescent histochemitry,glomerular expressions of nephrin and podocin were all significantly decreased in both groups,especially in IMV group.Conclusion A decrease in the expression of nephrin and podocin might participate in the development of type V lupus nephritis.
