淋巴细胞亚群不同凋亡在狼疮性肾炎患者发病中的意义

摘　　要：目的探讨狼疮性肾炎(LN)患者不同时期外周血单个核细胞凋亡率及CD3+CD4+、CD3+CD8+、CD3+CD4+Fas、CD3+CD8+Fas、sFas表达的意义。方法56例经肾活检证实的LN患者中,活动期38例,静止期18例,均采用酶联免疫吸附试验(ELISA)检测sFas和流式细胞仪检测PBMC凋亡率及CD3+CD4+、CD3+CD8+、CD3+CD4+Fas、CD3+CD8+Fas等指标。结果LN静止期sFas较健康对照组增高,活动期PBMC凋亡率、sFas明显升高(P<0.01),CD3+CD4+细胞较健康对照组明显减少(P<0.01)、CD3+CD8+细胞比健康对照组增多(P<0.01);PBMC凋亡率与sFas呈正相关。结论LN患者外周血T淋巴细胞的不同凋亡,特别是CD3+CD4+/CD3+CD8+比例失调,是导致LN病情发展的重要机制之一。sFas可能抑制肾小球系膜细胞凋亡,而且可能是LN进展中的一个因素。Objective To invstigate the clinical significance of apoptosis of peripheral blood mononuclear cells （PBMCs） and expression of CD3^＋ CD4^＋ , CD3^＋ CD8^＋ , CD3^＋ CD4^＋ Fas, CD3^＋ CD8^＋ Fas, sFas in patients with lupus nephritis （LN） during different periods. Methods Fifty-six patients with LN confired by kidney biopsy were enrolled in the investigation, in which 38 cases were in the active stage （active stage group）, and 18 cases were in the quiescent stage （quiescent stage group）. ELISA was applied to measuring sFas; while flow cytometry was adopted to detect the apoptotic rate of PBMCs and the expression levels of CD3^＋ CD4^＋ , CD3^＋CD8^＋, CD3^＋CD4^＋ Fas, and CD3^＋ CD8 ^＋ Fas. Results sFas was more elevated in quiescent stage group than that in healthy control group. The apoptotic rate of PBMCs, expression level of sFas and the level of CD3^＋ CD8^＋ cells were significantly higher, and the level of CD3^＋ CD4^＋ ceils was markedly declined in active stage group than those in healthy control group （P〈0.01）. The apoptotic rate of PBMCs was positively correlative to the expression level of sFas. Conclusion Differential apoptosis of lymphocyte subgroup, especially CD3^＋ CD4^＋/CD3^＋ CD8^＋ ratio imbalance, is one of the mechanisms of LN development. Inhibitory effect of sFas on the mesangial cells may be a factor for LN progression.
