吸入妥布霉素治疗铜绿假单胞菌下呼吸道感染的探讨

摘　　要：目的总结应用吸入妥布霉素治疗确定为铜绿假单胞菌引起的下呼吸道感染患者的临床效果,寻找较好的抗细菌的药物。方法选择60例随机分为治疗组和对照组,对照组根据细菌药敏试验选择抗生素头孢他定或左氧氟沙星。将头孢他定针剂2g溶于生理盐水100ml中,静脉点滴,每日2次;或左氧氟沙星针剂0.4g溶于生理盐水250ml中,静脉点滴,每日1次,疗程4周,并治疗基础疾病。治疗组在同对照组治疗的基础上,加用吸入妥布霉素0.4g加2ml生理盐水,用压力喷射雾化吸入器(德国百瑞)雾化吸入,2次/d,疗程6周。结果治疗组有效率为93.3%,对照组有效率为76.7%。治疗组和对照组对铜绿假单胞菌4周清除率分别为90%和66.7%。6周清除率分别为100%和70%。结论吸入妥布霉素治疗下呼吸道铜绿假单胞菌感染患者效果显著,未见明显毒副作用,患者病情迅速好转,病程缩短,对铜绿假单胞菌清除率高,时间越长清除率越高。Objective To sum up the clinical healing effect about inhaling tobramycin to treat pseudomonas aeruginosu Type of lower respiratory tract infection, and to seek for better antimicrobial drug. Methods To select 60 samples and divide them randomly into two groups ：remedial group and control group. To control group we select one of the antibiotics by drug sensitive test, the period of treatment is four weeks;To remedial group we add inhaling tobramycin, the period of treatment is six weeks. Results The effective rate is 93.3 % in remedial group, and in control group it is 76. 7%. In remedial group and control group the four weeks＇ clearance rate abour pseudomonas aeruginosu is 90% and 66.7%, and six weeks^learance rate is 100% and 70%. Conclusion The result of inhaling tobramycin to treat pseudomonas aeruginosu Type of lower respiratory tract infection is remarkable, and the harmful side effect of it is not obvious. By it patients＇ condition has been improved quickly, and the course of disease has been shortened, and the clearance rate is high, furthermore,the time is longer the clearance rate is higher.
