涤痰化瘀法对高血压左心室肥厚模型大鼠心肌细胞凋亡的影响

摘　　要：【目的】观察涤痰化瘀法对高血压左心室肥厚模型大鼠心肌细胞凋亡的影响。【方法】选用SPF级SD大鼠62只,随机分为5组:正常对照组,假手术组,模型组,中药低、高剂量组(分别按8.5、19.0.gkg-1.d-1剂量灌服涤痰化瘀方煎液);后3组均采用二肾二夹法复制高血压模型,手术35 d后各组按设计剂量连续给药60 d;观察各组血压、左室质量及全心质量,检测各组大鼠左心室质量指数(wLV),观察各组大鼠心肌病理形态变化,并采用流式细胞术检测各组心肌细胞凋亡率。【结果】中药低、高剂量组虽可降低血压、左室质量及全心质量,但与模型组比较无显著性差异(P>0.05);两中药组均可显著性降低左心室质量指数和心肌细胞凋亡率(P<0.05或P<0.01),改善心肌组织的病理变化,防止心肌坏死和纤维化。【结论】涤痰化瘀法对高血压的治疗作用可能与其能减少心肌细胞凋亡,逆转左心室重构,减少心肌坏死及纤维化有关。Objective To observe the effect of therapy of dispelling phlegm and resolving blood stasis on cardiac muscle cell apoptosis in hypertension rats with left ventricular hypertrophy. Methods Sixty-two SD rats were randomized into 5 groups： normal group, pseudo-operation group, model group, low- and high-dose herbal medicine groups （ treated with the decoction of phlegm-dispelling and stasis-resolving herbs at the doses of 8.5 and 19.0 g· kg^-1· d^-1 respectively）. The rat model of hypertension was induced by occlusion of bilateral renal arteries. Thirty-five days after operation, the rats received corresponding medication according to the experimental design. The treatment lasted 60 continuous days. Blood pressure, heart mass and the left ventrieular mass as well as left ventricular mass index were examined, the pathological changes of myoeardium was observed, and the apoptotie rate of cardiac muscle cells was detected by flow cytometry. Results Blood pressure, heart mass and the left ventricular mass were decreased in low- and high-dose herbal medicine groups, but the difference was insignificant as compared with the model group （P 〉 0. 05 ）. Left ventricular mass index and the apoptotic rate of cardiac muscle cells were decreased in low- and high-dose herbal medicine groups （ P 〈 0. 05 or P 〈 0. 01 compared with the model group）. Meanwhile, the pathological changes of myocardium were improved, and necrosis and fibrosis of myoeardium were reduced. Conclusion The therapeutic mechanism of therapy of dispelling phlegm and resolving blood stasis for hypertension may be related with the decrease of cardiac muscle cell apoptosis, reversion of left ventricular remodeling, and the reduction of myocardial necrosis and fibrosis.
