哮喘患儿CD8^+T淋巴细胞穿孔素与颗粒霉素B表达的研究

摘　　要：目的探讨CD8+T细胞穿孔素与颗粒霉素B的表达在儿童哮喘发病机制中的临床意义。方法测定121例哮喘患儿的外周血血清穿孔素与颗粒霉素B,其中哮喘发作组58例,男30例、女28例,平均年龄(5.91±2.84)岁;哮喘缓解组63例,男43例、女20例,平均年龄(7.16±3.00)岁;另测定正常对照组29例,男23例、女6例,平均年龄(5.90±1.01)岁。结果血清CD8+T细胞穿孔素与颗粒霉素B平均荧光强度表达在哮喘发作组及哮喘缓解组均明显高于正常对照组(P<0.001)哮喘发作组高于哮喘缓解组(P<0.05)。结论儿童哮喘是多细胞因子参与的气道慢性炎症性疾病,检测血清CD8+T细胞穿孔素及颗粒霉素B的表达有助于哮喘发病机制的深入研究,是否作为哮喘急性发病的检测指标有待进一步探讨。Objectives To explore the clinical significance of the expressions of granzyme B and perforin in cytotoxic T lymphocytes （CTL） in children with asthma. Methods 121 peripheral blood samples were collected from patients with asthma （including 58 with acute attack and 63 with controlled asthma）, and 29 healthy children were studied as control. There were 30 male and 28 female in the acute attack group, the mean age of this group was （5.91 ±2.84） years. There were 43 male and 20 female in the group with controlled asthma, the mean age of this group was （7.16 ±3.00） years. There were 23 male and 6 female in the normal control group, the mean age of this group was （5.90 ±1.01 ） years. Three-color flow cytometry was used for measurement of expression levels of granzyme B and perforin in CD3^＋ CD8^＋ lymphocytes. Results The expressions of granzyme B and perforin in CTL were significantly higher in the children both with acute attack and with controlled asthma than that of normal control （P 〈 0.001 ）, and they were both significantly higher in the children with acute attack than that of the group with controlled asthma （P 〈 0.05）. Conclusions Expressions of granzyme B and perforin were increased in asthma. CTL might be involved in the inflammation and contribute to asthma attacks. It should be further validated if those should be used as indexes of the acute phase.
