摘 要:目的通过观察视网膜脉络膜组织中MMP-2、MMP-9的表达变化,探讨归芍地黄汤、经瞳孔温热疗法(TTT)和联合疗法对脉络膜新生血管(CNV)的抑制作用。方法青紫蓝兔50只随机分为正常对照组、阴性对照组、TTT治疗组、中药治疗组、中药联合TTT治疗组。除正常对照组外,均用半导体激光行视网膜光凝建立CNV动物模型。造模后4周经FFA等检查证实有CNV后,分别给予不同的治疗,持续到8周,免疫组化法检测视网膜和脉络膜组织中MMP-2和MMP-9的表达。结果造模全部成功。①造模8周后各组视网膜脉络膜组织MMP-2和MMP-9的表达经单因素方差分析,组间差异有统计学意义(P<0.01)。②3个治疗组视网膜脉络膜组织中MMP-2和MMP-9的表达均明显低于阴性对照组(P<0.01)。③中药联合TTT治疗组MMP-2表达明显低于TTT治疗组(P<0.05)和中药治疗组(P<0.01);MMP-9表达明显低于中药治疗组(P<0.01),而与TTT治疗组比较则差异无统计学意义(P>0.05)。结论归芍地黄汤联合经瞳孔温热疗法可能通过调节视网膜脉络膜组织中MMP-2和MMP-9的表达,进而调控基质的降解与合成,从而在CNV的治疗中起抑制新生血管生成的作用。OBJECTIVE To investigate the inhibitory effect on choroidal neovascularization (CNV) of Guishao Dihuang decoction, Transpupillary thermotherapy (TTT) and combination therapy by detecting the expression changes of matrix metalloproteinases-2 (MMP-2) and matrix metalloproteinases-9 (MMP-9) in the retina and the choroid. METHODS Cyanotic blue rabbits were randomly divided into the normal control group, negative control group, TTT group, traditional Chinese medicine (TCM) group, TTT and TCM combination therapy group. Rabbits, excepting the rabbits in the normal control group, were administrated with laser photocoagulation using semiconductor laser to create the CNV animal model. Four weeks after modeling, CNV was confirmed by FFA. Then rabbits in the TTT group were administrated with TTT therapy; rabbits in the TCM group were administrated with Guishao Dihuang decoction; rabbits in the TIT and TCM combination therapy were administrated with TTT therapy and Guishao Dihuang decoction. Eight weeks after modeling, the expression of MMP-2 and MMP-9 in the retina and the choroid was detected by immunohistochemistry. RESULTS Four weeks of modeling, FFA showed CNV fluorescence leakage in the rabbits in the negative control group, the TTT group, the TCM group, TTT and TCM combination therapy group. Eight weeks of modeling, the differences in the expression of MMP-2 and MMP-9 in the retina and choroid among the five groups were statistically significanct (MMP-2: F= 86.57, P〈0.01; MMP-9: F=75.526, P〈0.01). The expression of MMP-2 and MMP-9 among the three treatment groups was lower than that in the negative control group (P〈0.01). The expression of MMP-2 in the TTT and TCM combination therapy was lower than that in the TTT group and the TCM group (P 〈0.01). The expression of MMP-9 in the TTT and TCM combination therapy was lower than that in the TCM group (P 〈0.01).
