摘  要:目的探讨数字减影CT血管成像(DSCTA)在血管性病变诊断中的应用价值。方法收集在本院行DSCTA检查的血管性疾病50例,使用西门子公司Emotion 16层螺旋CT机,螺距0.9~1.5,层厚0.75~1.5 mm,注药流率2.5~4.0 ml/s,成人剂量90~100 ml,儿童按2 ml/kg计算,扫描延迟时间用智能跟踪法扫描。图像后处理使用西门子公司Emotion工作站进行血管三维重建。结果DSCTA三维成像可以立体、直观地显示血管的空间解剖细节,可充分观察到动脉瘤的准确起源、瘤体大小、颈部宽窄及走向等情况,本组病例中所显示的最小动脉瘤直径约3 mm;同时,DSCTA也可作为动脉瘤术后评价的一种无创性检查方法,观察术后银夹所在的位置和远端血管通畅情况等。三维图像对于动静脉畸形、血管狭窄及颅内静脉系统疾病也具有良好的诊断价值。在重建成像中,容积再现图像质量最佳。结论DSCTA是一种无创性成像方法,在血管性疾病的诊断和术前术后评价中发挥着重要作用。Objective To evaluate the applied value of digital subtraction CT angiography (DSCTA) in diagnosis of vascular diseases. Methods 50 patients with vascular diseases were examined with Siemens Emotion 16 - slice helical CT. Pitch : 0.9 - 1.5, slice thickness : 0.75 - 1.5 mm, adult injection dosage : 90 - 100 ml, children injection dosage :2 ml/kg, injection rate :2.5 - 4.0 ml/s, delay time : Adopted intelligent track scan . Vascular images were processed at Siemens Emotion workstation. Results DSCTA could clearly display spacious anatomic details of aneurysm, including its origin, size, neck width, and trend etc. The diameter of the smallest aneurysm shown in our research was about 3 mm. As a non - invasive examination, DSCTA besides could be applied in post - operational evaluation of aneurysm by observing the location of silver clip and the distant vessels, could also be used to diagnose arteriovenous malformation,blood vessel narrow and cerebral vein diseases. Of all the imaging reconstruction methods, volume rendering ( VR ) is the best means to display the vascular diseases. Conclusion As a non - invasive examination , DSCTA plays an important role in detection , pre - operational and post - operational evaluation of vascular diseases.
