应用心肌功能指数评价风湿性二尖瓣狭窄患者左室心肌功能

摘　　要：[目的]探讨心肌功能指数(MPI)评价风湿性二尖瓣狭窄患者(MS)左室心肌功能的意义。[方法]选取MS患者及正常对照者各30例,首先测量左室射血分数(LVEF),然后应用定量组织速度成像技术(QTVI),测量左室各壁二尖瓣瓣环处的时间间期并计算MPI。[结果]MS患者LVEF与正常组比较差异无显著性意义(P>0.05),MS组左室等容收缩时间(ICT)、等容舒张时间(IRT)较正常组延长,差异有显著性意义(P<0.05;P<0.01),左室射血时间(ET)较正常组缩短,左室整体MPI较正常组增大,差异有显著性意义(P<0.01;P<0.01)。直线相关分析显示MPI与二尖瓣口面积(MVA)之间存在相关性(r=-0.35,P<0.01)。[结论]心肌功能指数(MPI)可以早期发现风湿性二尖瓣狭窄患者左室心肌整体功能的异常。[ Objective ] To investigate the value of myocardial performance index（MPI） in rheumatic mitral stenosis （MS） patients by quantitative tissue velocity imaging（QTVI）. [ Methods] Thirty MS patients and 30 healthy volunteers were enrolled. The left ventricular ejection fraction （LVEF） was measured firstly, the cardiac intervals were measured by QTVI and MPI was calculated finally. [ Results ] There was no significant difference of LVEF between two groups （ P 〉 0.05 ） ; ICT and IRT were longer than those in the normal group （P 〈0.05 ;P 〈0.01, respectively） ; ET was shorter than that in the normal group（P 〈0.01 ） ; MPI in the MS group was significant higher than that in the normal group（P 〈0.01 ）. MPI correlated with mitral valve area （r = -0. 35, P 〈 0.01 ）. [ Conclusion] MPI by QTVI can assess early abnormalities of myocardial performance in MS patients.
