摘  要:目的:针对影响放射性核素碘(131I)治疗Graves’病(GD)后近期甲状腺重量缩小率的相关影响因素进行综合性分析。方法:收集1 225例首次接受131I治疗的GD患者一般临床资料、相关的实验室检查资料以及甲状腺B超检查等资料,采用单因素方差分析、相关分析、多元线性回归等分析方法对其进行统计学分析。结果:(1)单因素方差分析结果显示:患者病情(F=9.665,P<0.001)、治疗前甲状腺重量(F=2.43,P=0.002<0.05)、服用131I剂量(F=1.66,P=0.042<0.05)等因素与治疗后近期甲状腺重量缩小程度有关。(2)计量资料的直线相关分析显示:治疗前甲状腺重量、有效半衰期、服用131I剂量等因素与治疗后近期甲状腺重量缩小率之间存在相关关系。(3)计量资料的多元线性回归分析显示:在131I治疗GD后近期甲状腺重量缩小率的相关影响因素中,治疗前甲状腺重量和有效半衰期是131I治疗后近期甲状腺重量缩小率的影响因素。结论:131I治疗GD后近期甲状腺重量缩小率受到多种因素的影响,在治疗前根据患者甲状腺重量和有效半衰期可以初步预测其治疗后甲状腺近期重量缩小程度。Objective: To analyze the correlation between early thyroid shrinkage rate after iodine radioisotope (^131I) therapy and its relevant factors in Graves' disease(GD)comprehensively. Methods: 1 225 GD cases received the first ^131I therapy were studied, whose general clinical data, results of laboratory test, and thyroid ultra sonograms were collected. Statistical analysis adopted included one-way analysis of variance, linear regression and linear correlation. Results: (1)One-way analysis of variance showed that early thyroid shrinkage rate associated with the seriousness of GD (F=9.665, P〈0.001), thyroid weight before treatment(F=2.43, P=0.002) and 131I dosage (F= 1 . 66, P=0.042) significantly. (2)Linear correlation analysis on quantitative data showed that there was strong relationship between early thyroid shrinkage rate and thyroid weight before treatment, effective half-life (EHL) and ^131I dosage. (3)Multifactor linear regression on quantitative data showed that thyroid weight before treatment and EHL were the most influential factors on early thyroid shrinkage rate after ^131I therapy of GD. Conclusion:Many factors can influence on thyroid shrinkage rate. Thyroid weight before treatment and EHL can be used as primary estimation on early thyroid shrinkage rate after ^131I therapy.
