体外循环心脏术后患者低钾的护理研究

摘　　要：目的了解体外循环围术期血清、尿液、骨骼肌以及红细胞中钾离子的变化规律,探讨合理的补钾用量和方法。方法选取低温体外循环下心脏直视术的病人47例。于围术期各时点测血常规、血清钾、红细胞钾浓度,同时测尿量、尿钾浓度。结果围术期红细胞内钾一直低于正常值;血清钾除停机及停机后15 m in时高于4.0mmol/L外,其余时间均低于4.0 mmol/L。结论转流期间每10分钟补钾0.134 mmol/kg的方式易引起肌体缺钾。适当增加转流初期钾的补充量有利于防治转流中、后的低钾血症。Objective：To explore the change of potassium in the body during cardiopulmonary bypass（CPB）and discuss the reasonable methods of potassium supply.Methods：Forty-seven patients undergoing open heart surgery with CPB were chosen randomly.Potassium concentrations in serun,urine and erythrocyte were measured.Results： potassium concentrations in erythrocyte were below the normal value in perioperation.Serun potassium concentrations were below4.0mmol/L in perioperation except during terminating CPB.Conclusion： During CPB0.749mmol/kg of potassium administered could not maintainserum potassium level over4.0mmol/L.
