血清总胆汁酸对肝胆疾病诊断的意义

摘　　要：目的研究血清总胆汁酸(TBA)对肝胆疾病诊断的价值。方法应用循环酶速率法对211例肝病患者及38例对照者的TBA水平进行检测,并同时测定其他肝功能指标。结果病例组血清TBA的平均水平明显高于对照组(P<0.01),并且肝硬化亚组和梗阻性黄疸亚组的TBA水平明显高于原发性肝癌亚组(P<0.01)。血清TBA的阳性率明显高于其他肝功能指标,血清TBA与总胆红素、直接胆红素有良好的相关性(P<0.01)。结论TBA是反映肝胆疾病较灵敏的生化指标,与胆汁代谢密切相关,对肝胆疾病的诊断具有重要的临床意义。Objective To explore diagnostic significance of serum total bile acid （TBA） in patients with hepatobiliary diseases. Methods The level of serum TBA was detected by cycle enzyme method in 211 patients with hepatobiliary diseases,including cirrhosis, obstructive jaundice, and primary liver cancer,and 38 control subjects. Other liver function parameters were also detennined. Results The level of TBA in patients with hepatobiliary diseases was significantly higher that in control subjects （P 〈 0.01 ）, and in patients with cirrhosis and obstructive jaundice it was significantly higher than in patients with primary liver cancer （P 〈 0.01 ）. In patients with hepatobiliary diseases, the positive rate of TBA was higher than other liver function parameters. The level of sennn TBA was significantly correlated with total bilirubin and direct bilirubin （P 〈 0.01 ）. Conclusion TBA is a sensitive biochemical index to reflect hepatobiliary diseases and closely correlated with bile metabolism. TBA plays an important role in the diagnosis of hepatobiliary diseases.
