糖尿病肢体动脉闭塞症中医证型与血流动力学及踝臂指数的相关性研究

摘　　要：目的探讨糖尿病肢体动脉闭塞症(DLAO)中医辨证分型与血流动力学及踝臂指数的相关性。方法将100例DLAO患者分为血瘀证组和湿热证组,每组各50例;同时选取20例健康体检者做为正常对照组。选取股总动脉的平均血流速度(Vm)、阻力指数(RI)、脉动指数(PI)、踝臂指数(ABI)、空腹血糖(FBG)、总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白(LDL)、高密度脂蛋白(HDL)、血栓素(TXB2)、6-酮-前列腺素(6-keto-PGF1α)、活化部分凝血活酶时间(APTT)及纤维蛋白原(Fib)等13项指标,运用Logistic回归分析方法,初步探讨其与中医证型的相关性。结果经统计分析认为PI、FBG、HDL和Fib等4项指标对DLAO中医证型贡献度最大,建立了最优回归预测方程。结论PI、FBG、HDL和Fib与DLAO中医证型关系密切,可作为评价DLAO中医辨证分型的参考指标。Objective To explore the correlation of Chinese medicine syndrome with hemodynamics and ankle-brachial index in diabetic limb arterial obliteration （DLAO）. Methods 100 cases of DLAO were divided into blood stasis syndrome group and damp heat syndrome group,50 cases in each one. Simuhaneously,20 cases of normal health were selected in normal control group. Logistic regression analysis was adopted to analyze 13 indexes,including mean velocity of common femoral artery （Vm of CFA）, resistance index （RI）, pulsing index （PI）, ankle-brachial index （ABI）, fasting blood glucose （FBG）, total cholesterol （TC）, triglycerides （TG）, LDL （low density lipoprotein）, HDL （high density lipoprotein）, thromboxane （TXB2）, 6-Keto-PGF1α, activated partial thromboplastin time （APTT）and fibringogen （Fib）to explore primarily the correlation of this disease with the syndromes in Chinese medicine. Results Statistical analysis viewed that PI, FBG,HDL and Fib had made the greatest contributions to DLAO syndromes in Chinese medicine;hence,the optimum regression equation was set up. Conclusion PI,FBG, HDL and Fib present close relations with DLAO syndromes in Chinese medicine and can be regarded as the reference indexes in assessment of the differentiation of DLAO syndromes in Chinese medicine.
