多普勒超声测舒张早期室内充盈减射评价左室舒张功能

摘　　要：应用多普勒测量正常人及扩张型心肌病和缺血性心肌病患者舒张早期血流速度在左室内的改变，以估测此种改变对判断左室舒张功能的价值。结果表明：正常组二尖瓣及瓣下１ｃｍ、２ｃｍ、３ｃｍ处心室内不同部位的舒张早期充盈速度（Ｅ０、Ｅ１、Ｅ２、Ｅ３）及各点与瓣尖速度比值（Ｅ１／Ｅ０、Ｅ２／Ｅ０、Ｅ３／Ｅ０）均无统计学差异。而扩张型心肌病和缺血性心肌病患者Ａ／Ｅ＞１、器质性房颤及Ａ／Ｅ＜１“假性正常化”分组中一般Ｅ２开始降低，Ｅ３显著降低Ｅ３／Ｅ０比值与Ｅ１／Ｅ０比较存在差异（Ｐ值＜０．０５；Ｐ值＜０．０１）。同时Ｅ３速度及Ｅ３／Ｅ０比值与对照组比较存在差异。作者认为多普勒测量舒张早期左室血流充盈减射可用于评价左室舒张功能。This study was designed to record mitral flow velocity (E0) and the intraventricular regional early diastolic flow velocity at 1 cm, 2 cm, 3 cm, from mitral tip toward the apex (E1, E2 and E3) in normal control group (18 individuals and 12 persons with Af only) and the heart disease group (29 with dilated cardiomyopathy, 16 ischemic cardiomyopathy) using Pulsed Doppler Echocardiography. In the control subjects the early distolic flow velocity at the mitral tip was maintained at the position 1 to 3 cm away from the tip into the LV cavity, in contrast. The early regional peak diastolic flow velocity progressively decreased from the tip toward the apex in patients with dilated cardiomyopathy and with ischemic cardiomyopathy. These fingings were also observed in patients with a nomalized mitral flow velocity pattern or those with Af. The assessment of the intraventricular decreese in peak early diastolic flow velocity may be useful in detecting LV diastolic dysfunction, particularly in patients with Af or a'normalized' mitral veiocity pattern.
