微创小切口全髋关节置换术60例临床体会

摘　　要：【目的】探讨微创小切口进行全髋关节置换手术的可行性和优越性。【方法】自2005年2月至2008年5月，应用小切口对60例患者68髋进行人工全髋关节置换手术，术后进行短期随访并做回顾性分析。【结果】68髋随访12～32周，平均18周，优53髋，良14髋，中1髋，优良率98．5％（67／68）。髋关节Harris评分由术前的平均36分提高到末次随访时的平均92分，复查X线片未见人工关节松脱，患者可负重行走，无一例出现严重并发症。【结论】微创小切口具有创伤小、出血量少、组织损伤少、医源性损伤少、人工关节稳定性强，以及患者恢复时间短等优势，是一种值得积极推广的手术方法。[Objective]To study the feasibility and advantage of techniques of minimal incision surgery for total hip arthroplasty（MIS- THA）. [Methods] From February 2005 to May 2008, 68 cases undergoing total hip arthroplasty with minimal incision received a short-term follow-up and were studied retrospectively. [Results]Sixty eight eases were followed up for 12 to 32 weeks ,with an average of 18 weeks. The excellent was 53, good 13,ordinary 1, and the rate of total fineness was 98.5%（67/68）. The mean Harris hip score was improved from 36 points preoperatively to 92 points at the last follow-up, and dislocated artificial joint was invisible by re-examining X-ray, and the patients were able to walk under heavy load without serious operative complications. [Conclusion]The minimal incision surgery is an ideal method to be popularized with less surgical trauma, less blood loss, less tissue injury, rapid postoperative recovery, and more stable artificial ioint.
