摘 要:[目的]采用表面增强激光解析离子化飞行时间质谱技术(SELDI-TOF-MS)筛选儿童哮喘血清蛋白标志物,建立一个稳定、简便、快速的实验方法。[方法]选择哮喘患儿、正常儿童各40例,按照年龄、性别1∶1配对,采用CM10芯片技术进行血清蛋白质组学研究,用蛋白芯片阅读器PBSⅡ对芯片进行扫描、分析。[结果]哮喘患儿血清蛋白图谱与正常儿童相比,存在15个差异表达蛋白(P<0.001),其中包括6个高表达蛋白,相对分子质量分别是5339、5639、5908、4718、5847、9300;9个低表达蛋白,相对分子质量分别是5164、3767、15314、3656、15135、14978、4178、5219、3488。明显高表达的潜在标志物2个,相对分子质量分别为5339、4718,以峰值比1.758为界值时,相对分子质量为5339的蛋白,诊断儿童哮喘的灵敏度为75.61%,特异度为72.05%;以峰值比2.258为界值时,相对分子质量为4718的蛋白,诊断儿童哮喘的灵敏度为70.73%,特异度为92.50%。选用5339、4718、3465、1006建立决策树模型对样品进行筛选验证,其准确率为75%,灵敏度为80%,特异度为70%。[结论]用该技术能筛选出灵敏度和特异度较好的差异表达蛋白,为儿童哮喘早期发现提供了一种新的实验方法,可能具有良好的临床应用前景。[ Objective ] To select the specific biomarkers in serum of children asthma using surface-enhanced laser desorption and ionization time of flight mass spectrometry( SELDI-TOF-MS )protein chip technology. [ Methods ] Serum samples from 40 children asthma patients, 40 healthy children with matched gender and age were analyzed using CM10 protein chip to screen potential biomarkers. Proteomic spectra were generated by mass spectrometry. [ Results ] Six high expressed potential hiomarkers were identified with the relative molecular weights of 5 339, 5 639, 5 908, 4 718, 5 847 and 9 300. Nine low expressed potential biomarkers were identified with the relative molecular weights of 5 164, 3 767, 15 314, 3 656, 15 135, 14978, 4178, 5 219 and 3 488. When the critical point was set as 1.758, the sensitivity of the protein 5 339 for diagnosing asthma was 75.61% and specificity was 72.05%. When the critical point was set as 2.258, the sensitivity of 5 339 for diagnosing asthma was 70.73% and specificity was 92.50%. The sensitivity for diagnosing asthma was 80% and the specificity was 70% by applying this protein fingerprint pattern validation. [ Conclusion ] SELDI-TOF-MS protein chip technology is a quick, easy, convenient and high- throughput analyzing method, which is capable of selecting several specific, potential biomarkers from the serum of children asthma patients. It may be used clinically.
