血管闭合装置Angio-seal在急诊冠脉介入治疗后股动脉止血中的应用

摘　　要：目的探讨急诊冠脉介入治疗术后股动脉穿刺处止血的理想方法。方法将80例急诊冠脉介入治疗术后患者随机分为两组:A组应用Angio-seal装置闭合股动脉穿刺点止血,B组应用Angio-seal装置闭合股动脉穿刺点加改良加压包扎法止血。两组抗血小板和抗凝治疗相同,比较两组止血成功率、并发症和患者的舒适度。结果①A组止血的成功率为100,B组止血的成功率为100,差异无显著性(P>0.05)。②主要并发症发生率:A组为45.0,B组为5.0,差异有非常显著性(P<0.01);③患者的舒适度:A组腰背疼痛10例(25.0),B组为0例。A组排尿困难5例(12.5),B组为1例(2.5),A组患者的舒适度明显低于B组,差异有显著性(P<0.05)。结论应用Angio-seal装置闭合动脉穿刺点加改良加压包扎法止血效果好,医疗费用无明显增加,值得临床推广。Objective The purpose of this study was to search an ideal method to seal femoral arterial puncture point after emergency pereutaneous coronary intervention. Methods Eighty patients undergoing emergency percutaneous coronary intervention in our department were selected in this study. The patients were randomly divided into 2 groups： group A and group B. The femoral artery puncture points in both groups were closed by Angio - Seal vascular closure device, while the improved pressure dressing was used additionally in group B. Both groups received anti - platelet and anticoagulant therapy. The differences in hemostasis efficiency, incidence rate of main complications and the degree of comfort between the two groups were compared. Results The hemostasis efficiency in both groups was 100%. The incidence rates of main complications were 45% and 5% in group A and group B respectively （ P 〈0.01 ）. There were 10 patients with lumbar and back pain in group A （25.0%） while the number was 0 in group B. Dysuresia occurred in 5 patients in group A and only in 1 patient in group B （ P 〈 0.05 ）. Conclusion The combined application of modified pressure dressing and Angio - Seal vascular closure device is more efficient for stopping bleeding without obvious increase in medical cost.
