摘 要:目的观察雾化吸入米力农对油酸诱导大鼠急性肺损伤(ALI)的影响。方法采用静脉注射油酸复制ALI大鼠模型。将20只SD大鼠随机分为两组:米力农雾化吸入治疗组(治疗组)和油酸诱导ALI组(对照组)。实验过程中每隔60min记录大鼠BP、肺动脉压(PAP)、气道压(Paw),并且检验动脉血气和混合静脉血气。注射油酸240min后检测支气管肺泡灌洗液(BALF)蛋白定量(TP)、白细胞计数及分类,测定肺湿干比(W/D)、肿瘤坏死因子α(TNF-α)浓度,并且对肺组织进行光镜和电镜观察。结果与对照组比较,治疗组PAP[(9.5±1.3)vs.(12.6±1.3)mmHg],Paw[(6.2±0.8)vs.(7.5±0.8)mmHg],肺内分流率[(13.5±3.3)%vs.(18.8±4.1)%]和氧合指数(PaO2/FiO2)[(300±26)vs.(226±17)mmHg]均明显改善,而治疗组TP[(0.55±0.07)vs.(0.71±0.1)g/L],白细胞计数[(0.66±0.1)vs.(1.11±0.09)×106/L]和分类[(29.4±3.1)%vs.(36.6±3.7)%],肺W/D[(6.2±0.4)vs.(7.2±0.4)],TNF-α[(213.7±11.5)vs.(296.3±15.7)ng/L)]均明显低于对照组(P<0.05)。光镜和电镜观察治疗组肺组织水肿以及板层小体脱颗粒现象得到明显改善。结论米力农雾化吸入能够缓解油酸引起的ALI。Objective To investigate the therapeutic effect of milrinone inhalation on oleic acid (OA)-induced acute lung injury (ALI) in rats. Methods ALl model was established by iv. injection of OA in 20 SD rats,which were randomly divided into milrinone inhalation group(treatment group) and control group with 10 rats each. Blood pressure (BP), pulmonary artery pressure (PAP), airway pressure (Paw), artery and mixed venous blood gas analysis was determined every 60 minutes during the experiment. Neutrophils and total protein (TP) content in hronchoalveolar lavage fluid (BALF), the ratio of wet to dry lung weight (W/D),and serum tumor necrosis factor (TNF)-α level were determined. Light and electron microscopy examination of lung specimens was performed after the experiment. Results Compared with control group,PAP[(9.5±1.3) vs. (12.6±1.3) mmHg],Paw [(6.2[±0.8) vs. (7.5±0.8) mm Hg],intrapulmonary shunt [(13.5±1-3.3) % vs. (18.8±4.1) %] and PaO2/FiO2[(300± 26) vs. (226± 17) mmHg] were all significantly improved in treatment group. The TP[ (0. 55 ±0. 07) vs. (0. 71± 0. ] ) g/L], neutrophils accountF(0. 66 ± 0. 1 ) vs. ( 1.11± 0. 09) × 10^6/L] and differentiation[(29.4±3.1)% vs. (36.6±3.7)%] in the lung W/D[(6.2±0. 4) vs. (7.2±0.4)],and TNF-α content[(213.7± 11.5) vs. (296.3±15.7) ng/L] were lower in treatment group than those in control group. Less serious lung injury on microscope examination was seen in treatment group than that in control group. Conclusion Milrinone inhalation can alleviate oleic acid-induced acute lung in jury in rats.
