超声心动图多巴酚丁胺负荷试验在非心脏手术患者术前心肌缺血评价中的意义

摘　　要：为评价超声心动图多巴酚丁胺负荷试验（ＤＳＥ）在非心脏手术患者术前心肌缺血评价中的意义，对２８例各种非心脏手术患者临床资料及ＤＳＥ结果进行了分析，男１３例，女１５例，平均年龄５９岁（２９～７５岁）。结果：可得出结论者２１例，其中阳性提示有心肌缺血者７例，占３３．３％，均首先接受了抗心肌缺血治疗。７例未达到预期心率，其中５例因不良反应而提前中止试验，而采用其他评价方法，全组围手术期无一例心肌梗塞或死亡发生。结论：ＤＳＥ是一种安全有效的术前心肌缺血评价手段。To assess the value of Dobutamine stress echocardiography(DSE)in detecting myocardial ischemia,we study the clinical informations and the results of DSE of 28 patients prior to non cardiac surgery.They were 13 male and 15 female with the mean age 59(29 75)years.21 cases had conclusive results,7 (33 3%)of them suggested myocardial ischemia,operation was postpone and anti ischemic treatment was scheduled.The negative DSE patients were operated.DSE was stopped at earlier stage due to poor durability or adverse effects in 7 patients.Two of them had their angiograms showing severe coronary artery disease and revascularization was done.The other 5 patients with normal left ventricular function were operated on.There was no perioperative myocardial infarction or death.Conclusion:DSE was very convenient,feasible,safe and non invasive to assess myocardial ischemia,adaptable for screening the high or low risk myocardial ischemia in patients undergoing non cardiac surgery operation.
