摘 要:目的了解多重耐药铜绿假单胞菌中β-内酰胺酶基因及膜孔蛋白基因存在状况,为临床合理使用抗菌药物提供理论依据。方法收集本院临床分离的铜绿假单胞菌共30株,采用纸片扩散法进行药敏试验;用聚合酶链反应(PCR)及序列分析方法,分析20种β-内酰胺酶基因(TEM、SHV、OXA-1群、OXA--2群、OXA-10群、PER、GES、VEB、CARB、IMI、VIM、SPM、GIM、SIM、DHA)和膜孔蛋白基因(OprD_2)。结果30株多重耐药铜绿假单胞菌中TEM基因阳性28株,阳性率93.3%。CARB基因阳性1株,阳性率3.3%;膜孔蛋白编码基因OprD_2缺失29株,阳性率96.7%。未检出其他β-内酰胺酶基因。结论烟台地区铜绿假单胞菌β-内酰胺酶基因(TEM)和膜孔蛋白编码基因OprD_2缺失株检出率均在90%以上。Objective To investigate the status of beta-lactamase genes and outer membrane protein gene in 30 strains of Pseudomonas aeruginosa (PA) isolated from clinical samples, so as to provide theoretical evidence for rational use of antibiotics in clinic. Methods Totally 30 strains of PA isolated from clinical samples were collected. Slip diffusing method was applied to performing drug susceptihility test, and polymerase chain reaction (PCR) assay and sequence analysis were adopted to investigate 20 kinds of beta-lactamase genes (TEM, SHV, OXA 1 group, OXA-10 group, PFR, GES, VEB, CARB, IMP, VIM, SPM, GIM, SIM and DHA) and outer membrane protein genes (OprD2). Results The positive rate of TEM, CARB and deletion of OprD2 gene was 93. 3% (28/ 30), 3.3% (1/30) and 96.7% (29/30) respectively. No other beta-lactamase gene was detected. Conclusion There is a higher positive rate (over 90%) of beta lactamase genes and outer membrane pro rein gene deficiency in Pseudomonas aeruginosa in Yantaioistrict.
