摘　　要：手术错误是手术室最大的风险之一,其发生的几率虽然不高,但后果严重,对病人和当事的医务人员均造成严重的伤害,也对医疗机构造成直接和间接的经济损失,产生不良的社会影响。故研究以医疗失效模式与效应分析(HFMEA)对手术流程进行分析,前瞻性地了解手术错误发生的过程及原因,再运用不同的质量改进方法制定对策,对于降低手术流程的风险有很大帮助。Surgical error is one of the biggest risk in operating room. Although the incidence rate is not high, but this would make some serious harm to the patients and medical staff, and result in some direct and indirect economic losses, adverse social impact to the medical institutions. In this prospective study, we used the Healthcare Failure Mode and Effect Analysis （HFMEA）, to analyze the surgical procedures, and realize the process and causes of surgical error. At last, we used some quality improvement methods to develop strategy, to reduce the risk of surgical procedures.
