益气活血、利湿降浊法治疗慢性肾衰竭远期疗效分析

摘　　要：目的:评价益气活血、利湿降浊法延缓慢性肾衰竭(CRF)病程进展的远期疗效。方法:采用回顾性临床病例研究方法,对84例以益气活血、利湿降浊法为主治疗36月以上CRF患者的病例资料进行综合分析。结果:治疗第6、36月,中医证候总积分及湿浊证、血瘀证证候积分.与治疗前比较,均明显减少(P<0.05)。治疗第3月时血清肌酐(Scr)、尿素氮(BUN)较治疗前明显下降(P<0.05)。治疗第6、12、24、36月,分别有73.97%、61.22%、33.33%、41.67%患者b值为正值。治疗第36月时,Scr倍增8例(9.5%)、终末期肾病(ESRD)6例(7.1%)、联合终点(Scr倍增、ESRD)10例(11.9%)。结论:益气活血、利湿降浊法能改善CRF(气虚血瘀、湿浊内停证)患者的中医证候,具有较好的近期疗效,还可延缓CRF病程进展。Objective:To observe the long-term therapeutic effect of therapy of strengthening Qi and activating blood,draining dampness and checking turbidity downward for chronic renal fail- ure(CRF).Methods:A retrospective trial was carried out in 84 CRF patients which received the therapy of strengthening Qi and activating blood,draining dampness and checking turbidity down- wind for over 36 months.Results:The total score of Chinese medical syndromes as well as the score of Qi-deficiency syndrome,damp-turbidity syndrome and blood-stasis syndrome was de- creased 6 and 36 months after treatment(P<0.03 compared with those before treatment).The serum creatinine(Scr)and blood urea nitrogen(BUN)levels were also decreased 3 months after treatment(P<0.03).The b value was positive in 73.97%,61.22%,33.33% and 41.67% of the patients 6,12,24 and 36 months after treatment,respectively.Doubling increase of SCr level occurred in 8(9.5%)patients,end-stage renal disease(ERSD)occurred in 6(7.1%)patients, and ESRD with doubling increase d Scr was found in 10(11.9%)patients.Conclusion:Therapy of strengthening Qi and activating blood,draining dampness and checking turbidity downward is ef- fective on improving syndromes of CRF patients with Qi deficiency and blood stasis,internal accu- mulatian of damp turbidity,which shows a good long-term therapeutic effect on counteracting the progress of CRF.
