二藤通痹合剂结合辨证中药治疗强直性脊柱炎的临床观察

摘　　要：目的:观察二藤通痹合剂结合辨证治疗强直性脊柱炎(AS)的-临床疗效。方法:90例强直性脊柱炎活动期的患者,随机分为2组,中药组60例,采用二藤通痹合剂联合辨证中药治疗;对照组30例,采用柳氮磺胺吡啶联合塞采昔布治疗。2组疗程均为12周。采用国际强直性脊柱炎评价组制定的ASAS20方案作为评价指标,以客观评价中医药治疗AS的疗效。结果:疗效满意率中药组为71.67%,西药组为60.00%。中药组和西药组治疗后躯体功能、脊柱痛、脊柱僵硬、病人总体评价、外周关节疼痛数均得到改善,其中躯体功能、脊柱痛、脊柱僵硬、病人总体评价、外周关节疼痛数中药组均优于西药组(P<0.05)。在第2周时,中药组与西药组同期比较,躯体功能、脊柱僵硬、外周关节疼痛数改善均优于西药组(P<0.05)。中药组和西药组治疗后胸廓活动度、Schober、指地距、枕墙距、脊柱侧屈、脊柱后仰均得到改善(P<0.05),且Schober、指地距、枕墙距、脊柱后仰中药组优于西药组(P<0.05)。在第2周时,中药组与西药组同期比较,胸廓活动度、Schober、指地距、脊柱后仰的改善均优于西药组(P<0.05)。中药组和西药组治疗后血沉、C反应蛋白均有下降,且中药组对血沉、C反应蛋白的改善从时间上、程度上均优于西药组(P<0.05)。在第2周时,中药组与西药组同期比较,血沉和C反应蛋白的下降均优于西药组(P<0.05)。结论:二藤通痹合剂联合辨证中药治疗强直性脊柱炎,能有效控制AS临床症状,改善机体免疫状况、减轻炎症,对早中期和部分晚期患者有良好的临床疗效。Objective:To observe the therapeutic effect of Erteng Tongbi Mixture(ETM)com- bined with syndrome differentiation and treatment for ankylosing spondylitis(AS).Methods: Nintey AS patients at active stage were randomized into 2 groups:the treatment group(n=60)re- ceived ETM combined with syndrome classification and treatment,and the control group(n=30) received sulfasalazine and celecoxib.The treatment course lasted 12 weeks.After treatment,the therapeutic effect was evaluated with the criteria of ASAS20.Results:The effective rate was 71.67% in the treatment group and 60.00% in the control group.After treatment,physical func- tion.rachialgia,back stiffness,patient general evaluation and the numher of painful peripheral joints were improved in the two groups,and improvement in the treatment group was superior to the control group(P<0.05).In the second week of treatment,the improvement of physical func- tion,back stiffness,and the number of painful peripheral joints was better in the treatment group than that in the control group(P<0.05).The thoracic activity,Schober's test,finger to floor distance,occiput to wall distance,spinal lateral curvature,and spinal hypsokinesis were improved in the two groups after treatment(P<0.05),and the improvement of Schober's test,finger to floor distance,occiput to wall distance,and spinal hypsokinesis in the treatment group was superior to that in the control group(P<0.05).In the second week of treatment,the improvement of tho- racic activity,Scheber's test,finger to floor distance,end spinal hypsokinesis were better in the treatment group than that in the control group(P<0.05).The erythrocyte sndimentation rate (ESR)and C-reactive protein level were decreased in the two groups,and the decrease in the treatment group was superior to that in the control group after treatment and in the second week of treatment(P<0.05).Conclusion:Erteng Tongbi Mixture(ETM)combined with syndrome clas- sification and treatment is effective for ankylosing spondylitis at early and middle stage and for som
