从手术证实的腰椎间盘突出症的分型谈治疗问题(附110例分析)

摘　　要：方法：手术治疗１１０例腰椎间盘突出症，根据术中所见髓核突出情况分为膨出型２１例、突出型８２例、脱出型７例。结果：术后随访半年～６年，疗效优良率达８９０９％．结论：作者认为，膨出型及大部分突出型腰椎间盘突出症神经根的机械压迫及继发病变轻，椎管的有效容量存在，保守治疗效果较好。部分突出型、脱出型腰椎间盘突出症神经根的机械压迫及继发病变较重，椎管的有效容积较小，保守治疗只能缓解症状，或症状不解除需手术治疗。术中彻底解除赖以解释神经根受损的病变，术后充分引流，手术往往取得满意的效果。Methods:According to the protrusion of necleus pulposus in the surgery,110cases of the lumbar intervertebral disc can be divided into 3 types:21 cases of protruded type,82 cases of herniated type,7 cases of prolapsus type.Results:To followup after 672 months,the curative effect 89% are good.Conclusion:In spite of the symptoms of the mechanical pressure on the nerve root of the protrudedtype and of the greater part of the herniatedtype lumbar intervertebral disc,the secondary diseases were not serious,the effectire of the spinal canal existed.Therefore,nonsurgical treatment would lead to better results.However,because of the symptoms of the mechanical pressure on the nerve root of the part of the herniatedtype lumbar interverbral disc and of the prolapsustype lumbar interverbral disc,the secondary diseases became more serious,the effectire of the spinal canal was smaller.Therefore,the nonsurgery treatment would only relieve the symptoms,and sometimes,the symptoms even still existed as usual.In this condition,the surgery was needed for the case.The injured nerve root,diagnosed in the clinical examination,could be completely cured during surgery and the lesion could be full discharged after surgery.So satisfactory results would often be got.
