摘 要:目的探讨利多卡因局部湿敷对减轻桡动脉穿刺疼痛的有效方法。方法选择符合病例标准的桡动脉穿刺患者100例,随机分为两组,两组首次穿刺采用传统法,第二次穿刺采用配制2%利多卡因无菌棉片湿敷法,实验组在桡动脉穿刺前10 min湿敷,对照组在桡动脉穿刺前5 min湿敷,采用视觉模拟量表(VAS)和Wong-baker面部表情量表,分别对两组患者不同表面麻醉时间桡动脉穿刺的镇痛效果进行评估,并对患者发放"无痛穿刺满意度调查表"进行认可性及可行性调查。结果两组比较,实验组镇痛视觉模拟量表(VAS)评分和Wong-baker面部表情量表评分都显著降低(P<0.001),桡动脉穿刺前10 min采用2%利多卡因浸湿的无菌棉片湿敷镇痛效果明显优于穿刺前5 min湿敷,镇痛效果显著(P<0.001),一次穿刺成功率提高(P<0.05),患者总体满意度提高(P<0.001)。结论表面麻醉防治桡动脉穿刺疼痛效果好,适宜临床应用。Objective To study the effect of analgesic by local Lidocaine wet spread, and propose the effective method of preventing ache of radial artery puncture. Method 100 patients with radial artery puncture was selected, and randomly divided into two groups. Each group was 50 patients. The first puncture of the two groups was using the traditional way, the second radial artery puncture in experimental group was applied the self-made 2 % Lidocaine aseptic cotton and kapok piece to spread wet for 10 minutes, the control group was applied spreads wet in the first 5 minutes of radial artery puncture. Visual simulation meter (VAS) and Wong-baker facial expression meter was used to estimate analgesic effect in the different topical anesthesia time. the indolence puncture degree of satisfaction survey form was used survey the survey feasibility. Result The analgesic effect by Visual simulation meter (VAS) and Wong-baker facial expression meter in experimental group was significantly reduce (P〈0. 001), the analgesic effect of using the aseptic cotton and kapok piece content2% Lidocaine spreading for 10minutes before the radial artery puncture is higher than that of wet spread in 5 minutes before the puncture (P〈0. 001). The ratio of single puncture's success and patient's overall satisfaction was improved (P〈0.05, P〈0. 001). Conclusion The effect of topical anesthesia was effective for preventing pain during radial artery puncture, and It's good. for clinical practice
