清热化痰法对COPD急性加重期患者细胞因子的影响

摘　　要：目的观察清热化痰法对慢性阻塞性肺疾病(COPD)急性加重期患者血清肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)水平的影响。方法将100例COPD急性加重期中医证属痰热郁肺型患者随机分为两组,对照组采用WHO发布的COPD规范化治疗方案,治疗组在此基础上加用具有清热化痰作用的中药,两组疗程均为7d;比较两组治疗后血TNF-α、IL-6水平的变化及临床疗效。结果两组治疗后TNF-α、IL-6水平均较治疗前明显下降,但治疗组下降幅度大于对照组;治疗组临床控制率高于对照组。结论清热化痰法治疗COPD急性加重期患者,可以更明显的降低血清TNF-α、IL-6水平,减轻炎症反应,提高临床疗效。Objective： To observe the curative effect on acute exacerbations of chronic obstructive pulmonary disease （AECOPD） with the management of clearing away heat - evil and resolving phlegm by analyzing the levels of plasma tumor necrosis factor- α（TNF-α） and interleukin-6（IL-6） in the blood. Methods： One hundred hospitalized AECOPD patients with retention of phlegm and heat syndrome in the lung in TCM were randomly divided into control group and treatment group. The control group accepted the treatment of COPD norms WHO issued by the chronic obstructive pulmonary disease global Initiative. The treatment group was given the Chinese medicine which have the efficacy of clearing away heat - evil and resolving phlegm, and simultaneously accepted conventional therapy of the western medicine same as the control group. The period of treatment was 7d. To compare the result of two groups before and after treatment by analyzing the level of TNF - α, IL - 6 in the blood. Results： The level of TNF - α, IL - 6 in two groups were reduced after treatment, the treatment group reduced more than control group, and effective rate was higher than control group. Conclusion： The management of clearing away heat - evil and resolving phlegm can treat AECOPD with SIRS. It can reduce blood TNF -α, IL - 6 levels and inflammatory response obviously. It can improve the efficacy.
