摘　　要：目的:评价Tamm-Horsfall(THP)免疫荧光染色法和相差显微镜法在肾性血尿诊断中的意义。方法:对诊断明确的30例肾小球性血尿和25例非肾性血尿同时进行尿红细胞THP免疫荧光检测和相差显微镜形态检测,比较两种方法在确定血尿来源的差异,并对血尿严重程度与尿白蛋白排泄率(UAER)做相关分析。结果:在30例肾小球性血尿中,红细胞THP检测法确定肾小球性血尿28例,2例与诊断不符;在25例非肾小球性血尿中,红细胞THP检测法确定为非肾小球性血尿22例,3例与诊断不符。红细胞THP检测法灵敏度为93.3%、特异性为88.0%、准确性为90.9%、漏诊率为6.7%、误诊率为12.0%。尿红细胞的形态检测法敏感性为80.0%、特异性为72.0%、准确性为66.3%、漏诊率为20.0%、误诊率为28.0%。血尿严重程度与肾脏损害程度不相关。结论:尿红细胞THP免疫荧光法在检测血尿来源方面具有准确、客观的优点,可在临床推广使用。Objective： To evaluate the significance in the diagnosis of kidney hematuria by.urine erythroeytes THP immunofluorescence assay and phase contrastmicroscope. Methods：A clear diagnosis of 30 cases of glomerular hematuria and 25 af non glomerular hematuria were examined by THP immunofluoreseence assay and phase contrast microscope to compare the two methods in determining the source of hematuria. We also do correlation analysis between the extent of urine erythrocytes and urilmry albumin excretion rate（UAER）.Results：28 glomerular hematuria were identified among 30cases through detection of urine Tamm-Horsfall protein coated erythrocytes and 2 were misjudged, meanwhile 22 nongiomerular hematuria were identified among 25 cases and 3 were not confirmed. The method was 93.3% in sensitivity,88.0% in specificity and 90.9% in accuracy,and the rate of under diagnosis was 6.7% and the rate of misdiagnosis was 12.0%. The erythrocyte morphology method in differentiation of hematuria origin was 80.0% in sensitivity. 72.0% in specificity and 66.3 % in accuracy . and the underdiagnosis rate was 20.0% and the misdiagnosis rate was 28.0%.Hematuria seriousness and the extent of kidney damage are not relevant.Conclusion：Inmmnofluorescenee staining of urine erythrocytes THP protein in identifition sources of hematuria apper to be stable. accurate, objective, reliable. In order to diagnosis and treatment of hematuria patients early.we can gradully extend this method to clinical work.
