摘  要:目的确定D-二聚体定量结果应用于深静脉血栓(DVT)诊断的临界值,并重新评价其临床意义。方法按照DVT的诊断标准收集患者标本237例,以同期检测的其他疾病患者303例为对照。免疫比浊法测定D-二聚体含量,根据结果绘制ROC曲线,并应用统计软件进行分析。结果根据ROC曲线,D-二聚体用于DVT的最佳诊断临界值为1.98μg/mL(敏感度为51.48%,特异性为62.61%,阳性似然比为1.38)。结论以1.98μg/mL作为D-二聚体诊断DVT的临界值较为合适,可提高其临床诊断的效率。Objective To determine the cut-off value of D-dimer in the diagnosis of deep vein thrombosis(DVT) with ROC curve and reappraise its clinical significance. Method Blood samples of 237 cases patients with DVT were collected, and so were those of 303 control cases. All these blood samples were measured for D-dimer content with immunoturbidimetric methods, and then ROC curve was drawn and the cut-off value was determined with statistical software. Results The most appropriate D-dimer cut-off value in the diagnosis of DVT was 1.98 μg/mL(with sensitivity 51.48%, specificity 62.61% and likelihood ratio 1.38). Conclusion Using 1.98 μg/mL as the cut-off value in the diagnosis of deep vein thrombosis would improve the efficiency of the diagnosis.
