加味四君子汤治疗脾气虚型常年性变应性鼻炎的临床疗效研究

摘　　要：目的研究加味四君子汤对脾气虚型常年性变应性鼻炎患者的治疗作用。方法脾气虚型常年性变应性鼻炎患者60例,随机分为实验组、西药对照组,每组各30例;另设健康对照组30例。观察2组治疗前后总体疗效、血清IL-4、IL-12及IFN-γ值变化有无显著性差异。结果①临床疗效上,实验组显著优于西药对照组(P<0.05);②治疗前实验组、西药对照组血清IL-4明显高于健康对照组,IFN-γ、IL-12明显低于健康对照组,(P<0.01);③治疗后,实验组、西药对照组血清IL-4均下降,血清IFN-γ均上升,实验组明显优于西药对照组(P<0.01);实验组治疗后血清IL-12明显升高,治疗前后有非常显著的差异(P<0.01),西药对照组血清IL-12治疗前后无显著差异(P>0.05)。结论加味四君子汤对脾气虚型常年性变应性鼻炎具有较好的临床疗效,临床上值得推广。Objective To investigate the therapeutic effect of Supplemented Four Gentlemen Decoction （DGD） on perennial allergic rhinitis （PAR） in a Spleen-Qi deficiency pattern （SQDP） based on a clinical trial. Methods Included in this study were 60 cases with PAR in SQDP, randomly divided into treating group （TG） treated by DGD and controlling group （CG） treated only by chemical drugs, with 30 cases in each group. Furthermore, serum samples were taken from these cases before and after the therapy respectively to detect the changes in the levels of IL-4, IL-12 and IFN-7 in serum. Then, comparisons were made to analyze the differences in the total effective rates and the serum level changes in IL-4, IL-12 and IFN-7 between the two groups, with 30 healthy persons as the normal references （NR） for the detection of serum IL-4, IL-12 and IFN% Results The therapeutic effect of TG was significantly superior to that of CG, with marked statistical significance between them （/9〈0.05）. As compared with that of NR, the serum level oflL-4 was significantly higher in both of TG and CG, while the levels oflL-12 and 1FN-7 were obviously lower than that ofNR （P〈0.01） before the therapy. By the end of therapeutic course, the sertun level of IL-4 was lowered and that of IFN-3＇ was elevated in both of TG and CG, however, with significantly higher changing levels among the cases ofTG （P〈0.01）. On the other hand, there was a significant increase in the serum level oflL-12 among the cases ofTG （P〈0.01） when compared before and after the treatment, while no such a change in this indicator （P〉0.05） among the cases of CG following the therapy. Conclusions Supplemented Four Gentlemen Decoction holds significant therapeutic effect on perennial allergic rhinitis in the pattern of Spleen-Qi deficiency and it is worthwhile for this medicine to be promoted into practical usage in the treatment of perennial allergic rhinitis.
