摘  要:目的研究不同损伤部位对大鼠动眼神经功能修复的影响与机制。方法健康SD大鼠40只均分4组,正常对照组(10只),动眼神经不损伤,其余30只分为3组,第1实验组10只在海绵窦后段、第2实验组10只在海绵窦中段、第3实验组10只在眶尖部锐性切断动眼神经并将断端对齐固定,术后动态观察神经功能恢复情况并行动眼神经系统解剖、组织学研究。结果术后在动眼神经眶尖段神经切断实验组大鼠眼外肌功能恢复较好,在海绵窦中段动眼神经切断组大鼠眼外肌功能恢复次之,在海绵窦后段神经切断实验组大鼠眼外肌功能恢复最差。3个实验组中支配眼外肌的中脑躯体运动神经元总数量无显著差别(P>0.05),在眶尖段神经切断组大鼠新生神经纤维对上直肌支配的特异性最高,在海绵窦神经切断组次之,在海绵窦后段神经切断组最差(P<0.01)。结论动眼神经损伤部位离靶器官越近神经功能恢复越好,其中决定因素可能是新生神经纤维对靶器官的特异性支配程度。Objective To study the influence and mechanism of different position injury on oculomotor nerve functional recovery. Methods Forty Sprague Dawley (SD) rats were divided into 4 groups: 10 rats without injury were kept as normal control group, the other 30 rats were divided into 3 groups ( each n = 10) with oculomotor nerve cut at different positions : at post-cavernous sinus in group 1 ; at middle-cavernous sinus in group 2 and at oculomotor opex through craniotomy by inicrosurgery: all the nerve stumps were sutured properly. The anatomy and histology of the oculomotor nerve were studied after operation and its functional recovery of the regenerated nerve was observed. Results Functional recovery of extraocular muscle as well as superior rectus between muscle fibrins and their innervating neurons where the nerve was cut in orbital opex was better than the rats with nerve cut at post cavernous sinus and cavernous sinus ( P 〈 0.01 ). The total number of somatic motility neurons in diencephalons had no differences in three groups ( P 〉 0.05 ). Conclusion The closer the injury position was to the target organ, the better the recovery of oculomotor nerve function was. The key factor might be the degree of the specific innervation from the newborn neurofibers to the target organ.
