摘 要:目的观察重型肝炎患者T细胞亚群、白介素18(IL-18)、γ干扰素(INF-γ)表达水平,探讨其与重型肝炎发病机制的关系。方法抽取重型肝炎18例、慢性肝炎13例、正常健康志愿者9例清晨空腹静脉血5 mL,其中2 mL抗凝快速送检T细胞亚群,另外3 mL新鲜分离血清-80℃保存,整批用酶联免疫吸附实验(enzyme-linked im-munosorbent assay,ELISA)方法检测IL-18和IFN-γ水平。结果重型肝炎与慢性肝炎患者、正常人相比总CD3+、CD4+/CD3+细胞水平、CD4+/CD8+下降,CD8+/CD3+细胞水平升高;IL-18、INF-γ水平升高,且二者显著相关。结论重型肝炎患者细胞免疫功能紊乱,细胞因子在重型肝炎的发病机制中起一定的作用。Objective To investigate the levels of T cell subgroup,interleukin 18 ( IL-18 ) anu lnterferon γ (IFN-γ) among the patients with severe hepatitis and their role in pathogenesis of severe hepatitis. Methods Fasting 5 mL venous blood was obtained in the morning from 1 8 patients with severe hepatitis, 13 patients with chronic hepatitis and 9 healthy volunteers. The levels of T cell subgroup were measured in 2 mL antieoagulated blood, and the other 3 mL blood was stored in - 80 ℃ for detecting levels of IL-18 and IFN-γ/by enzyme-linked immunosorbent assay (ELISA). Results In severe hepatitis, the levels of CD3 ^+ ,CD4^+/CD3 ^+ , CD4^+/CD8 ^+decreased, levels of CD3 ^+/CD8 ^+ increased, levels of IL-18 and IFN- γrose compared with chronic hepatitis patients and healthy controls, the differences were significantly. Conclusion There is relationship between immunity disturbance and severe hepatitis, and cytokines play certain role in the pathogenesis of severe hepatitis.
