下腹部“Y”型蒂皮瓣修复手部撕脱伤

摘　　要：目的介绍“Y”型蒂下腹部皮瓣在修复手部撕脱伤中的应用。方法以腹股沟韧带中点与脐部连线为轴线．设计下腹部“Y”型蒂皮瓣，长宽比为1—1．5：1。由皮瓣远端向近端掀起，再以皮瓣远端的中点为起点，沿轴线切剪成两叶皮瓣，分别缝合皮瓣和两叶皮瓣近端皮管为蒂，将皮瓣转移至创面处缝合封闭创面。结果术后皮瓣全部成活，创面一期修复，手部外形良好，并最大限度地保留了伤指长度。结论下腹部“Y”型蒂皮瓣设计简单．手术简便，术后形态良好，是修复手指撕脱伤或手部小面积皮肤缺损的一种可供选择的方法。Objective To introduce application of ＂Y＂-shaped pedicel skin flap in lower abdomen for repairing hand avulsion. Method ＂Y＂-shaped pedicel skin flap in lower abdomen was designed by the line of inguinal ligament middle point and umbilical part, its length/breadth ratio was 1-1.5/1. The pedicel skin was cut into two parts uniformly and was transferred to avulsion area. Result All the flaps survived with satisfactory .function and appearance. Conclusion The procedure is simple, the survival rate is high, the procedure is a appropriate method for repairing hand avulsion.
